预警性干预模式联合心电监护仪监测对急性肝衰竭短期预后生存分析

Short-term prognostic survival analysis of an early warning intervention model combined with cardiac monitor monitoring in acute liver failure

目的探讨预警性干预模式联合心电监护仪监测对急性肝衰竭患者短期预后生存的影响。方法选取2022年1月-2023年1月天津市第三中心医院收治的89例急性肝衰竭患者纳入研究,按照随机数字表法分为对照组(n=44,常规护理联合心电监护仪监测)和观察组(n=45,在对照组基础上采用预警性干预模式)。评估对比两组患者的肝功能指标、短期预后情况及生存质量。结果干预前,两组研究对象总胆汁酸(TBA)、谷草转氨酶(AST)、谷丙转氨酶(ALT)、血清总胆红素(TBiL)水平、生存质量(WHO-QOL-BREF)评分对比,无统计学差异(P>0.05);干预后,观察组TBA、AST、ALT、TBiL水平低于对照组,干预后4、12、24周WHO-QOL-BREF评分高于对照组,差异有统计学意义(P<0.05);观察组不良预后率(11.11%)低于对照组(28.89%),差异有统计学意义(P<0.05)。结论预警性干预模式联合心电监护仪监测有助于改善急性肝衰竭患者的肝功能和短期预后,提高患者生存质量,具有临床应用价值。

Objective To explore the effect of early warning intervention mode combined with cardiac monitor monitoring on the shortterm prognosis of acute liver failure patients'survival.Methods 89 patients with acute liver failure admitted to our hospital from January 2022 to January 2023 were selected to be included in this study,and were divided into the control group(n=44,routine care combined with cardiac monitor monitoring)and the observation group(n=45,based on the early warning intervention mode in the control group)according to the randomized numerical table method.The liver function indexes,short-term prognosis and survival quality of patients in the two groups were assessed and compared.Results Before intervention,there was no statistically significant difference in the comparison of total bile acids(TBA),azelaic transaminase(AST),alanine transaminase(ALT),serum total bilirubin(TBiL)levels,and quality of survival(WHO-QOL-BREF)scores of the study subjects in the two groups(P>0.05);after intervention,the TBA,AST,ALT,and TBiL levels of the observation group were lower than those of the control group,after 4,12,24 weeks WHO-QOL-BREF scores were higher than those of the control group(P<0.05);the adverse prognosis rate of the observation group(11.11%)was lower than that of the control group(28.89%)(P<0.05).Conclusion Early warning intervention mode combined with cardiac monitor monitoring helps to improve the liver function and short-term prognosis of patients with acute liver failure,improve the quality of patient survival,and has clinical application value.
