心肾综合征患者营养不良现状及影响因素分析

Analysis of malnutrition status and influencing factors in patients with cardiorenal syndrome

目的调查心肾综合征(CRS)患者营养不良现状,并分析其影响因素。方法采用便利抽样法选取2023年3月至2023年8月于中国医学科学院阜外医院肾内科病房住院或肾内科门诊就诊的CRS患者作为调查对象。使用一般资料调查表、微型营养评估量表(MNA)、抑郁症筛查量表、临床资料调查表进行调查。采用单因素分析和二元logistic回归模型分析数据。结果共纳入148例CRS患者,存在营养不良的患者有44例,患病率为29.7%,存在营养不良风险的患者有75例,占50.7%。二元Logistic回归结果显示N末端B型利钠肽原(NT-proBNP)、血红蛋白浓度、血肌酐水平及抑郁与CRS患者营养不良的发生有关(P<0.05)。结论CRS患者营养不良患病率及其发生风险均较高,临床医护人员应关注患者营养状况,及时发现营养不良及其影响因素,并实施个性化营养干预。

Objective To investigate the status of malnutrition in patients with cardiorenal syndrome(CRS)and analyze its influencing factors.Methods Patients with the diagnosis of CRS who were hospitalized in the nephrology ward or visited the nephrology outpatient department of Fuwai Hospital,Chinese Academy of Medical Sciences,from March 2023 to August 2023 were selected as the investigation objects by convenience sampling method.General information questionnaire,Mini-Nutritional Assessment(MNA),Patient Health Questionnaire-9(PHQ-9),clinical data questionnaire were used.Multivariate logistic regression model were used to evaluate the influencing factors.Results A total of 148 patients with CRS were enrolled.The prevalence of malnutrition in patients with CRS was 29.7%(44/148),and the patients with nutritional risk accounted for 50.7%(75/148).Binary logistic regression analysis showed that N terminal-pro B type natriuretic peptide(NT-proBNP),hemoglobin concentration,serum creatinine and depression are associated with malnutrition in patients with CRS(all P<0.05).Conclusion The prevalence rate and risk of malnutrition in patients with CRS are high.Clinical staff should pay attention to the nutritional status of patients,timely discover malnutrition and its influencing factors,and implement personalized nutritional intervention.
