增强T_(2) FLAIR-T_(1)WI强化不匹配特征预测弥漫性胶质瘤IDH突变状态的价值

The Predictive Value of Enhanced T_(2) FLAIR-T_(1)WI Mismatch Feature in Predicting IDH Mutation Status in Diffuse Glioma

摘要 目的 探讨增强T_(2)FLAIR-T_(1)WI强化不匹配特征预测弥漫性胶质瘤IDH突变状态的诊断价值。方法 分析经病理确诊的65例弥漫性胶质瘤患者术前MRI增强T_(1)WI、增强T_(2)FLAIR强化特征,比较IDH突变组(27例)与野生组(38例)弥漫性胶质瘤增强T_(2)FLAIR-T_(1)WI强化不匹配比例的差异。结果 IDH突变组与野生组的增强T_(1)WI强化程度、比例有统计学差异(P<0.05),IDH突变组与野生组在增强T_(2)FLAIR的强化程度、比例有统计学差异(P<0.05),IDH突变组与野生组在增强T_(2)FLAIR-T_(1)WI强化不匹配比例上差异具有统计学意义(P<0.05)。对IDH突变状态的预测,强化不匹配比例的敏感度、特异度、AUC值分别为81.6%、62.0%、0.767。结论 增强T_(2)FLAIR-T_(1)WI强化不匹配特征有助于术前预测弥漫性胶质瘤的IDH突变状态。 Objective To investigate the diagnostic value of enhanced T_(2) fluid-attenuated inversion-recovery(FLAIR)-T_(1)WI mismatch feature in predicting isocitrate dehydrogenase genes(IDH)mutation status in diffuse glioma.Methods 65 cases of diffuse glioma confirmed by histopathology were analyzed for enhancement features on preoperative MRI contrastenhanced(CE)-T_(1)WI and CE-T_(2) FLAIR.The difference in the proportion of enhanced T_(2) FLAIR-T_(1)WI mismatch between IDH mutation(27 cases)and wild-type(38 cases)diffuse gliomas was compared.Results There were statistically significant differences(P<0.05)in the enhancement quality and proportion enhancing on CE-T_(1)WI and CE-T_(2) FLAIR between the IDH mutation and wild-type groups.The difference in the proportion of enhanced T_(2) FLAIR-T_(1)WI mismatch was statistically significant(P<0.05)between the two groups.For predicting IDH mutation status,the sensitivity,specificity,and AUC value of the proportion of enhanced mismatch were 81.6%,62.0%,and 0.767,respectively.Conclusion Enhanced T_(2) FLAIR-T_(1)WI mismatch feature can help predict IDH mutation status in diffuse glioma preoperatively.
