新生血管性青光眼患者内皮素-1、趋化因子受体7、转化生长因子-β2、白细胞介素-6水平变化及其相关性分析

Analysis of changes in serum levels of endothelin-1,CXC-domain chemokine receptor 7,transforming growth factor-β2,interleukin-6 and their correlation in patients with neovascular glaucoma

摘要 目的探讨新生血管性青光眼患者血清内皮素-1、趋化因子受体7(CXCR7)、转化生长因子-β2(TGF-β2)、白细胞介素-6(IL-6)水平变化及其相关性。方法本研究为前瞻性队列研究,选取2019年6月至2023年8月西电集团医院眼科收治的82例新生血管性青光眼患者作为病例组,男43例,女39例,年龄(55.49±4.32)岁,年龄范围为34~68岁。根据其临床分期情况分为Ⅰ期组(n=25)、Ⅱ期组(n=30)和Ⅲ期组(n=27)。另选取同期西电集团医院眼科收治的110例年龄相关性白内障患者作为对照组,男57例,女53例,年龄(55.37±4.41)岁,年龄范围为33~68岁。比较病例组与对照组、不同临床分期新生血管性青光眼患者的内皮素-1、CXCR7、TGF-β2、IL-6水平,采用Pearson相关性分析新生血管性青光眼患者血清内皮素-1、CXCR7、TGF-β2、IL-6水平之间的相关性,绘制受试者操作特征(ROC)曲线分析血清内皮素-1、CXCR7、TGF-β2、IL-6单一及联合检测对新生血管性青光眼的预测价值。结果Pearson相关性分析结果显示,新生血管性青光眼患者血清内皮素-1水平与血清CXCR7、TGF-β2、IL-6水平呈正相关(r=0.513、0.492、0.486,P<0.05);血清CXCR7水平与血清TGF-β2、IL-6水平呈正相关(r=0.526、0.489,P<0.05);血清TGF-β2水平与血清IL-6水平呈正相关(r=0.504,P<0.05)。ROC曲线分析结果显示,血清内皮素-1、CXCR7、TGF-β2、IL-6联合检测预测新生血管性青光眼的曲线下面积为0.916,高于内皮素-1、CXCR7、TGF-β2、IL-6单一检测(0.797、0.830、0.724、0.819,P<0.05)。结论内皮素-1、CXCR7、TGF-β2、IL-6在新生血管性青光眼患者中呈高表达,四者可参与患者疾病进展过程,且四者联合检测预测新生血管性青光眼的价值更高。 Objective To investigate the changes in serum levels of endothelin-1(ET-1),CXC-domain chemokine receptor 7(CXCR7),transforming growth factor-β2(TGF-β2),and interleukin-6(IL-6)and their correlation in patients with neovascular glaucoma.Methods The study was a prospective cohort study,a total of 82 patients with neovascular glaucoma admitted to department of Ophthalmology in Xidian Group Hospital from June 2019 to August 2023 were selected as the case group,including 43 males and 39 females,aged(55.49±4.32)years old,the age ranging from 34 to 68 years old,which were divided into the stageⅠgroup(n=25),stageⅡgroup(n=30),and stageⅢgroup(n=27)according to their clinical stages,and 110 patients with age-related cataract admitted to department of Ophthalmology in Xidian Group Hospital during the same period were selected as the control group,including 57 males and 53 females,aged(55.37±4.41)years old,the age ranging from 33 to 68 years old.The serum levels of ET-1,CXCR7,TGF-β2,and IL-6 were compared between the case and control groups,and patients with neovascular glaucoma of different clinical stages,and the correlation between serum levels of ET-1,CXCR7,TGF-β2,and IL-6 of patients with neovascular glaucoma was analyzed by Pearson correlation analysis,and the predictive value of serum ET-1,CXCR7,TGF-β2,IL-6 single and combined tests for neovascular glaucoma was analyzed by receiver operator characteristic(ROC)curve.Results The Pearson's correlation analysis showed that the level of serum ET-1 in patients with neovascular glaucoma were positively correlated with serum levels of CXCR7,TGF-β2 and IL-6 were positively correlated(r=0.513,0.492,0.486,P<0.05);serum level of CXCR7 were positively correlated with serum levels of TGF-β2 and IL-6(r=0.526,0.489,P<0.05);serum level of TGF-β2 were positively correlated with serum level of IL-6(r=0.504,P<0.05).ROC curve analysis showed that the area under the curve value for the combined serum ET-1,CXCR7,TGF-β2,and IL-6 test to predict neovascular glaucoma was 0.916,which was higher than those of the single test for serum ET-1,CXCR7,TGF-β2,and IL-6(0.797,0.830,0.724,0.819,P<0.05).Conclusions Serum ET-1,CXCR7,TGF-β2,and IL-6 were highly expressed in patients with neovascular glaucoma,and all four could be involved in the process of disease progression in patients,and the value of combined detection of all four was higher in predicting neovascular glaucoma.
