球囊扩张联合支架植入术对下肢动脉硬化闭塞症患者功能恢复及微循环状态的影响

Effect of Balloon Dilation Combined with Stent Implantation on Functional Recovery and Microcirculation in Patients with Lower Extremity Arteriosclerosis Obliterans

目的探索球囊扩张联合支架植入术对下肢动脉硬化闭塞症患者功能恢复及微循环状态的影响。方法选取2019年10月~2022年10月我院血管外科收治动脉粥样硬化闭塞症患者72例,按随机数字表法分为对照组和观察组,对照组35人;观察组37人;对照组采取单纯球囊扩张术;观察组采取球囊扩张术联合支架植入术。结果观察组临床总有效率97.30%高于对照组74.29%,差异具有统计学意义(P<0.05);治疗前两组跛行距离、ABI、Rutherford分比较差异无统计学意义(P>0.05);治疗后,两组Rutherford分较治疗前下降,跛行距离、ABI分较治疗后升高(P<0.05);观察组Rutherford低于对照组,跛行距离、ABI高于对照组,差异有统计学意义(P<0.05);治疗前两组足管襻形态、血液流态及襻周状态比较差异无统计学意义(P>0.05);治疗后,两组管襻形态、血液流态及襻周状态较治疗前下降(P<0.05);观察组管襻形态、血液流态及襻周状态低于对照组,差异有统计学意义(P<0.05)结论球囊扩张联合支架植入术对下肢动脉硬化闭塞症患者不仅能达到更好的疗效,有益于患者功能恢复并且能更好的改善患者微循环状态降低远期再狭窄风险。

Objective To explore the effect of balloon dilation combined with stent implantation on functional recovery and microcirculation in patients with lower extremity arteriosclerosis obliterans.Methods Seventy-two patients with lower extremity arteriosclerosis obliterans treated in Vascular Surgery of our hospital from October 2019 to October 2022 were enrolled,and classified into two groups according to random number table methods.Control group(n=35)received balloon dilation only,while observation group(n=37)received balloon dilation combined with stent implantation.Results The clinical efficacy rate was 97.30%in observation group,which was higher than 74.29%in control group,with statistical difference(P<0.05).The ankle-brachial index(ABI),claudication distance and Rutherford grade yielded no statistical difference between two groups before treatment(P>0.05),while a decrease in Rutherford along with an increase in ABI and claudication distance were observed in both groups after treatment(P<0.05).Observation group had lower Rutherford,and higher ABI and claudication distance than those of control group after treatment(P<0.05).The capillary loop score,blood flowing state and perivascular tissue score demonstrated no statistical difference between two groups before treatment(P>0.05).The above three parameters decreased in both groups after treatment,and were lower in observation group than in control group(P<0.05).Conclusion Application of balloon dilation combined with stent implantation in the treatment of lower extremity arteriosclerosis obliterans can effectively promote the functional recovery,improve the microcirculation and reduce the risk of long-term restenosis in patients.
