脑颈一体化超声结合血清NSE对急性脑梗死神经功能预后的预测研究

摘要 目的:本研究旨在评估脑颈一体化超声联合血清神经元特异性烯醇化酶(NSE)在预测急性脑梗死(ACI)患者神经功能预后不良方面的价值。方法:纳入207例ACI患者和207例健康志愿者,进行脑颈一体化超声检查,测量相关血流参数和颈动脉特征,并采用电化学免疫发光法检测血清NSE水平。通过随访统计神经功能预后不良发生率,并对相关因素进行Logistic回归分析,应用受试者操作特征(ROC)曲线分析脑颈一体化超声、血清NSE对ACI患者神经功能预后不良的预测价值。结果:与健康志愿者相比,ACI患者的脑颈一体化超声参数显著异常,血清NSE水平升高。神经功能预后不良的患者在多个指标上表现不佳。联合使用脑颈一体化超声和血清NSE提高了对神经功能预后不良的预测准确性。结论:脑颈一体化超声结合血清NSE是预测ACI患者神经功能预后不良的有效方法,具有较高的灵敏度和准确性。研究贡献:本研究为临床医生早期识别神经功能预后不良的ACI高危患者提供了一种新的可靠的预测工具,从而指导临床治疗和改善预后。同时,该研究也丰富了急性脑梗死预后评估领域的研究内容,为未来的研究提供了新的思路和方向。 Objective:The aim of this study was to evaluate the value of integrated brain-neck ultrasound combined with serum neuron-specific enolase(NSE)in predicting poor neurological outcomes in patients with acute cerebral infarction(ACI).Methods:A total of 207 patients with ACI and 207 healthy volunteers were included in the study.Integrated brain and neck ultrasonography was performed to measure the blood flow parameters and carotid artery characteristics,and serum NSE levels were detected by electrochemical immunoluminescence method.The incidence of adverse neurological function prognosis was analyzed through follow-up,and relevant factors were analyzed by Logistic regression.The predictive value of cerebrocervical integrated ultrasound and serum NSE for adverse neurological function prognosis in ACI patients was analyzed by receiver operating characteristic(ROC)curve.Results:Compared with healthy volunteers,ACI patients showed significant abnormalities in brain neck integrated ultrasound parameters and elevated serum NSE levels.Patients with poor neurological prognosis perform poorly on multiple indicators.The combined use of integrated brain neck ultrasound and serum NSE improved the accuracy of predicting poor neurological function prognosis.Conclusion:The combination of brain-neck ultrasound and serum NSE is an effective method to predict the poor prognosis of neurological function in ACI patients,with high sensitivity and accuracy.Contribution:This study provides clinicians with a new and reliable predictive tool for early identification of high-risk patients with ACI with poor neurological function prognosis,thereby guiding clinical treatment and improving prognosis.At the same time,this study also enriched the research content in the field of acute cerebral infarction prognosis assessment,and provided new ideas and directions for future research.
