摘要
目的:探讨狭窄牙槽嵴中种植体舌侧平移后垂直或舌倾10°对种植体周围骨组织应力分布的影响。方法:建立狭窄牙槽嵴中种植体舌侧平移1 mm后垂直或舌倾10°植入的三维有限元模型为实验组;正中垂直植入为对照组。施加载荷并分析周围骨组织内应力分布。结果:种植体正中垂直植入时骨组织内应力最小最均匀,其次为舌侧平移后垂直、舌侧平移后舌倾。颊向舌载荷下种植体舌侧平移后舌倾比舌侧平移后垂直植入颈部颊侧皮质骨内拉应力峰值增大1.67倍、松质骨内增大0.99倍。结论:骨量充足垂直植入最利于种植体周围骨组织应力分布;狭窄牙槽嵴中种植体舌侧平移后垂直或舌倾10°对植体周围骨组织应力分布影响较小,可作为狭窄牙槽嵴种植方案选择的参考。
Objective:To research stress distribution in narrow residual ridge when inserted implants in lingual offset location,with a vertical or 10˚lingually inclined direction by finite element analysis.Methods:A three-dimensional finite element model of a narrow alveolar ridge implant with a lingual translation of 1 mm and a vertical or lingual inclination of 10˚°implantation was established as the experimental group.Control group(model A)was inserting the implant vertically in the abundant residual ridge and experimental groups were placing the implant in lingual offset of 1 mm with a vertical(model B)or 10˚lingually inclined direction(model C).Loads were applied to the crown and stress distributions in peri-implant bone were analyzed.Results:Under the loads,stress in peri-implant bone of model A was the smallest,followed by model B and C.Compared with model B,the maximum tensile stress in cervical cortical bone and cervical cancellous bone of model C increased by 1.67 and 0.99 times respectively.Conclusion:It was the most beneficial to the stress distribution of peri-implant bone by inserting implant vertically in the mandibular posterior region with abundant residual ridge.Stress of peri-implant bone increased slightly when inserting implants in lingual offset location,with a vertical or 10˚lingually inclined direction where available bone width was insufficient,which can be used as a choice for implant placement in narrow alveolar ridge.
