体温专项干预对高龄髋关节置换患者术后发热寒战、麻醉复苏及凝血功能的影响

Effect of special intervention of body temperature on fever and chills,anesthesia recovery and coagulation function in elderly patients after hip replacement

摘要 目的 分析体温专项干预对高龄髋关节置换术患者术后发热寒战、麻醉复苏及凝血功能的影响。方法 选择2021年5月~2022年5月在我院行髋关节置换术的90例高龄患者,随机分为对照组(n=45)与研究组(n=45)。对照组接受常规体温干预,研究组接受体温专项干预,对两组术后发热发生率、寒战发生率、麻醉苏醒时间、自主呼吸恢复时间、呼之睁眼时间、拔管时间及手术前后凝血功能指标进行比较。结果 研究组术后发热发生率6.67%,寒战发生率8.89%,均显著低于对照组26.67%,33.33%(P<0.05)。研究组麻醉苏醒时间(20.55±2.16)min、自主呼吸恢复时间(8.57±1.84)min、呼之睁眼时间(13.92±1.88)min及拔管时间(27.43±3.45)min均较对照组(25.67±3.17)min、(9.66±1.52)min、(15.14±1.75)min、(32.71±3.68)min缩短(P<0.05)。与对照组比较,研究组术后的凝血酶原时间(PT)、活化部分凝血酶原时间(aPTT)、凝血酶时间(TT)、纤维原蛋白(FIB)均较低(P<0.05)。结论 加强体温专项干预有利于降低高龄髋关节置换术患者术后发热寒战发生率,维持患者机体凝血功能稳定,缩短麻醉苏醒时间。 Objective To analyze the effect of special intervention of body temperature on fever and chills,anesthesia recovery and coagulation function in elderly patients undergoing hip replacement.Methods A total of 90 elderly patients who underwent hip replace-ment in our hospital from May 2021 to May 2022 were selected as research samples and randomly divided into control group(n=45)and study group(n=45).The control group received routine body temperature intervention,while the study group received special body tempera-ture intervention.The incidence of fever,incidence of shivering,anesthesia recovery time,spontaneous breathing recovery time,eye opening time,extubation time and coagulation function indexes before and after operation were compared between the two groups.Results The inci-dence of fever was 6.67%and chills was 8.89%in the study group were significantly lower than those in the control group 26.67%,33.33%(P<0.05).The anesthesia recovery time,spontaneous breathing recovery time,eye opening time and extubation time of the study group were(20.55±2.16)min,(8.57±1.84)min,(13.92±1.88)min and(27.43±3.45)min respectively,which were shorter than those of the control group(25.67±3.17)min and(9.66±1.52)min,(15.14±1.75)min,(32.71±3.68)min(P<0.05).Compared with the control group,the prothrombin time(PT),activated partial prothrombin time(aPTT),thrombin time(TT)and fibrinogen protein(FIB)in the study group were lower after operation(P<0.05).Conclusion Strengthening the special intervention of body temperature is beneficial to reduce the incidence of fever and chills after hip replacement in elderly patients,maintain the stability of coagulation function,and shorten the recovery time from anesthesia.
