尿中性粒细胞明胶酶相关脂质运载蛋白在糖尿病肾病诊断中的应用价值

Application Value of Urinary Neutrophil Gelatinase-Associated Lipocalin in the Diagnosis of Diabetic Nephropathies

目的探究尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)在糖尿病肾病(DN)诊断中的应用价值。方法选取2020年在武汉科技大学附属天佑医院住院或门诊确诊的2型糖尿病患者90例为研究对象,根据是否发生DN将其分为DN组(n=40)和非DN组(n=50)。比较两组一般资料、血液检查指标〔空腹血糖(FBG)、糖化血红蛋白(HbA_(1c))、胱抑素C(Cys-C)、血尿素氮(BUN)、血肌酐(Scr)、TC、TG、LDL-C、HDL-C及极低密度脂蛋白胆固醇(VLDL-C)〕、尿液检查指标〔尿蛋白质、尿葡萄糖、尿白细胞、尿酮体及尿NGAL(采用His-tag胶体金快速检测卡进行定性分析)〕。结果DN组FBG、HbA_(1c)、Cys-C、BUN、TG高于非DN组(P<0.05)。两组尿蛋白质、尿葡萄糖、尿白细胞检测结果比较,差异有统计学意义(P<0.05)。尿NGAL诊断DN的灵敏度为65.0%,特异度为96.0%,正确率为82.2%。结论尿NGAL对DN具有一定诊断价值,可能为DN的早期筛查提供一定帮助。

Objective To investigate the application value of urinary neutrophil gelatinase-associated lipocalin(NGAL)in the diagnosis of diabetic nephropathies(DN).Methods A total of 90 patients with type 2 diabetes who admitted or were diagnosed in the Outpatient Department of Tianyou Hospital Affiliated to Wuhan University of Science and Technology in 2020 were selected as the research objects.According to whether DN occurred,they were divided into DN group(n=40)and non-DN group(n=50).The general data,blood test indicators[fasting blood glucose(FBG),glycosylated hemoglobin(HbA_(1c)),cystatin C(Cys-C),blood urea nitrogen(BUN),serum creatinine(Scr),TC,TG,LDL-C,HDL-C and very low density lipoprotein cholesterol(VLDL-C)],urine examination indexes[urinary protein,urinary glucose,urinary white blood cells,urinary ketone body and urinary NGAL(qualitative analysis through His-tag colloidal gold rapid detection card)].Results FBG,HbA_(1c),Cys-C,BUN and TG in the DN group were higher than those in the non-DN group(P<0.05).There were significant differences in urinary protein,urinary glucose and urinary white blood cells between the two groups(P<0.05).The sensitivity,specificity and accuracy of urinary albumin in the diagnosis of DN were 65.0%,96.0%and 82.2%,respectively.Conclusion Urinary NGAL has a certain diagnostic value for DN,which may provide some help for the early screening of DN.
