母亲心音环境联合袋鼠式干预对早产儿生长发育、胆红素代谢和心肺功能的影响

Effects of the mother's heart sound environment combined with the kangaroo mother care intervention for premature infants on their growth and development, bilirubin metabolism and cardiopulmonary function

摘要 目的:分析母亲心音环境联合袋鼠式干预对早产儿生长发育、胆红素代谢和心肺功能的影响。方法:选取本院2019年1月-2022年1月收治的早产儿96例,随机数字表法分为48例对照组(常规护理模式)和48例观察组(母亲心音环境联合袋鼠式干预护理模式)。比较两组胆红素代谢、心肺功能、睡眠质量、生长发育情况及产妇护理满意度。结果:两组经皮胆红素值在第24 h、72 h、120 h、168 h呈先上升后下降趋势,第120h达到峰值,且观察组[(72.35±11.21、123.69±12.47、152.36±28.69、86.15.±9.12)μmol/L]均低于对照组[(78.68±12.69、129.23±13.10、177.58±35.52、91.14±11.02)μmol/L],在干预第4、7天两组心率和呼吸频率逐渐降低,血氧饱和度逐渐升高,且观察组变化幅度大于对照组(均P<0.05);在干预第28天,观察组夜间觉醒次数(4.45±0.98次)低于对照组(5.38±1.03次),夜间睡眠时间(10.85±1.38h)和白天睡眠时间(9.24±1.25h)高于对照组(10.12±1.24h、8.63±1.46h),头围(3.12±0.56cm)、身长(5.12±1.25cm)及体重(621.64±101.19g)均高于对照组(2.88±0.48cm、4.45±1.31cm、538.55±92.14g),总护理满意度(95.8%)高于对照组(83.3%)(均P<0.05)。结论:母亲心音环境联合袋鼠式护理模式能有效降低早产儿胆红素水平,改善心肺功能和睡眠质量,促进生长发育,提高家属护理满意度。 Objective:To analyze the effects of the mother heart sound environment combined with the kangaroo mother care intervention for premature infants on their growth and development,bilirubin metabolism and cardiopulmonary function.Methods:96 premature infants who had been treated in the hospital were selected and were divided intothe control group(48 cases with the routine nursing model) and the observation group(48 cases with the indirect mother heart sound environment combined with the kangaroo mother care intervention model) by the random number table method from January 2019 to January 2022.The bilirubin metabolism situation,the cardiopulmonary function,the sleep quality and the growth and development status of the infants,and the maternal satisfaction for the nursing were compared between the two groups.Results:The transcutaneous bilirubin value of the infants in the two groups had increased firstly and then had decreased at 24h,72h,120th and 168h after intervention,and which(72.35±11.21μmol/L,123.69±2.47μmol/L,152.36±28.69μmol/L and 86.15±9.12μmol/L) of the infants in the observation group were significantly lower than those(78.68±12.69μmol/L,129.23±13.10μmol/L,177.58±35.52μmol/L and 91.14±11.02μmol/L) of the infants in the control group,and the transcutaneous bilirubin value of the infants in the two groups reached the peak at 120h after birth.The heart rate and respiratory rate of the infants in the two groups on the4th and 7th day of intervention had decreased gradually,and the blood oxygen saturation value of the infants increased gradually,and the changes of which in the observation group were significantly greater than those of the infants in the control group(all P<0.05).The number of the night awakening(4.45±0.98 times) of the infants in the observation group on the 28th day of intervention was significantly lower than that(5.38±1.03times) of the infants in the control group,and the night sleep time(10.85±1.33h) and daytime sleep time(9.24±1.25h) of the infants in the observation group on the 28th day of intervention were significantly higher than those(10.12±1.24h and 8.63±1.46h) of the infants in the control group.The values of head circumference(3.12±0.56cm),length(5.12±1.25cm) and weight(621.64±101.79g) of the infants in the observation group were significantly higher than those(2.88±0.48cm,4.45±1.31cm and 538.55±92.14g) of the infants in the control group.The total maternal nursing satisfaction(95.8%) in the control group was significantly higher than that(83.3%) in the control group(all P<0.05).Conclusion:The mother's heart sound environment combined with kangaroo mother care model for the premature infants can effectively reduce their bilirubin level,improve their cardiopulmonary function and sleep quality,promote their growth and development and enhance the nursing satisfaction of their family members.
