维生素D联合阿奇霉素对儿童肺炎支原体肺炎的治疗效果

Therapeutic effect of vitamin D combined with azithromycin on Mycoplasma pneumonia in children

目的探讨维生素D联合阿奇霉素治疗儿童肺炎支原体肺炎(MP)的效果。方法队列研究。抽取2020年9月至2023年9月郑州大学附属儿童医院收治的MP患儿112例,按照随机数字表法分为阿奇霉素组和阿奇霉素联合维生素D组,每组56例。阿奇霉素组采用阿奇霉素治疗,阿奇霉素联合维生素D组在阿奇霉素组基础上加用维生素D治疗。比较两组临床疗效、临床症状消失时间;比较两组治疗前后炎症因子[白细胞介素-3(IL-3)、C反应蛋白(CRP)、可溶性细胞间黏附分子-1(sICAM-1)]水平、肺功能[用力肺活量(FVC)、1秒用力呼气容积(FEV1)]。结果阿奇霉素联合维生素D组总有效率(96.43%,54/56)高于阿奇霉素组(78.57%,44/56),P<0.05。阿奇霉素联合维生素D组临床症状消失时间短于阿奇霉素组(P<0.05)。治疗后,阿奇霉素联合维生素D组IL-3、CRP、sICAM-1水平低于阿奇霉素组(P<0.05)。治疗后,阿奇霉素联合维生素D组FVC、FEV1水平高于阿奇霉素组(P<0.05)。结论维生素D联合阿奇霉素治疗儿童MP效果显著,能改善炎症反应,减轻临床症状,提高肺功能。

Objective To investigate the therapeutic value of vitamin D combined with azithromycin in the treatment of Mycoplasma pneumonia(MP)in children.Methods A total of 112 children with MP treated in Children’s Hospital Affiliated of Zhengzhou University from September 2020 to September 2023 were selected for the cohort study.And they were divided into the azithromycin group and the azithromycin combined with vitamin D group by random number table method,with 56 cases in each group.The azithromycin group was treated with azithromycin,and the azithromycin combined with vitamin D group was treated with vitamin D based on the treatment of the azithromycin group.The curative effect and disappearance time of clinical symptoms of the two groups were compared.The levels of inflammatory factors,including interleukin-3(IL-3),C-reactive protein(CRP)and soluble intercellular adhesion molecules-1(sICAM-1),and pulmonary function evaluated by forced vital capacity(FVC)and forced expiratory volume in 1 second(FEV1)were compared between the two groups before and after treatment.Results The total effective rate of the azithromycin combined with vitamin D group(96.43%,54/56)was higher than that of the azithromycin group(78.57%,44/56),P<0.05.Compared with the azithromycin group,the disappearance time of clinical symptoms in the azithromycin combined with vitamin D group were shorter(P<0.05).After treatment,the azithromycin combined with vitamin D group had lower levels of IL-3,CRP and sICAM-1 compared with the azithromycin group(P<0.05).Compared with the azithromycin group,the azithromycin combined with vitamin D group had significantly higher levels of FVC and FEV1 after treatment(P<0.05).Conclusions The effect of vitamin D combined with azithromycin in the treatment of MP in children is significant,which can improve inflammatory response,reduce clinical symptoms,and improve lung function.
