儿童血清禁食诱导脂肪细胞因子表达水平及意义

Expression level and significance of adipocytokines induced by serum fastingin obese children

摘要 目的探究儿童血清禁食诱导脂肪细胞因子(FIAF)表达水平与肥胖发生及程度的关系。方法收集191例肥胖症儿童和同期健康体检的191例同龄儿分别为研究组和对照组。测定两组受试儿血清FIAF,分析其与肥胖发生、肥胖程度和机体代谢状态的关系。结果研究组体重、腰臀比(WHR)、高密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、内脏脂肪指数(VAI)、胰岛素抵抗指数(HOMA-IR)、超敏C反应蛋白(hs-CRP)、FIAF水平均明显大于对照组,脂蛋白脂酶(LPL)明显低于对照组(P<0.05)。重度肥胖儿体重、WHR、LDL-C、TG、VAI、HOMA-IR、hs-CRP和FIAF明显高于中度肥胖儿,中度肥胖儿明显高于轻度肥胖儿;重度肥胖儿血清LPL水平明显低于中度肥胖儿,中度肥胖儿明显低于轻度肥胖儿(P<0.05);代谢异常肥胖儿体重、WHR、LDL-C、TG、VAI、HOMA-IR、hs-CRP、LPL和FIAF水平明显高于代谢正常肥胖儿,血清LPL水平明显低于代谢正常肥胖儿(P<0.05)。相关性分析显示,肥胖儿血清FIAF水平与体重、WHR、LDL-C、TG、VAI、HOMA-IR、hs-CRP水平均呈正相关,与LPL呈负相关(P<0.05)。结论儿童体内FIAF上调表达水平与肥胖症发生、发展密切相关,其血清水平不仅可反映儿童肥胖程度,而且有助于区分代谢健康型肥胖和代谢异常型肥胖。 Objective To explore the relationship between the expression level of fasting-induced adipocytokines(FIAF)in serum and the occurrence and degree of obesity in obese children.Methods 191 obese children and 191 healthy children of the same age were collected as the study group and the control group respectively.Serum FIAF was measured in two groups,and its relationship with obesity,obesity degree and metabolic state of the body was analyzed.Results The body weight,waist-hip ratio(WHR),high-density lipoprotein cholesterol(LDL-C),triglyceride(TG),visceral fat index(VAI),insulin resistance index(HOMA-IR),high-sensitivity C-reactive protein(hs-CRP)and FIAF levels in the study group were significantly higher than those in the control group,while lipoprotein lipase(LPL)was significantly lower than that in the control group(P<0.05).The weight,WHR,LDL-C,TG,VAI,HOMA-IR,hs-CRP and FIAF of severely obese children were significantly higher than those of moderately obese children,and the above indexes of moderately obese children were significantly higher than those of mildly obese children,and the serum LPL level of severely obese children was significantly lower than that of moderately obese children,and the LPL level of moderately obese children was significantly lower than that of mildly obese children(P<0.05).The levels of body weight,WHR,LDL-C,TG,VAI,HOMA-IR,hs-CRP,LPL and FIAF in metabolically abnormal obese children were significantly higher than those in metabolically healthy obese children,and the serum LPL level was significantly lower than that in metabolically healthy obese children(P<0.05).Correlation analysis showed that the serum FIAF level was positively correlated with body weight,WHR,LDL-C,TG,VAI,HOMA-IR and hs-CRP levels,and negatively correlated with LPL(P<0.05).Conclusion The up-regulated expression level of FIAF in children is closely related to the occurrence and development of obesity,and its serum level can not only reflect the degree of obesity in children,but also help to distinguish between metabolically healthy obesity and metabolically abnormal obesity.
