超声引导下高位胸神经节射频治疗带状疱疹后神经痛临床观察

Clinical Observation of Ultrasound-guided High Thoracic Ganglion Radiofrequency Therapy for Postherpetic Neuralgia

目的:评价超声引导下高位胸神经节射频治疗带状疱疹后神经痛的临床疗效及安全性。方法:选择2021年1月—2023年1月于北京市顺义区医院确诊并接受治疗的带状疱疹后神经痛患者85例作为研究对象,按照随机数字表法分为对照组和治疗组。对照组42例,采用普瑞巴林胶囊治疗;治疗组43例,在对照组治疗方法基础上配合超声引导下高位胸神经节射频治疗。分别在治疗前、治疗后1周、1个月、3个月,比较两组视觉模拟评分(VAS)、总有效率及不良反应发生情况。结果:治疗前,两组VAS评分比较,差异无统计学意义(P>0.05);治疗1周、1个月、3个月后,两组VAS评分低于治疗前,治疗组低于对照组,差异有统计学意义(P<0.05)。治疗组总有效率(86.05%)高于对照组(66.67%),差异有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:与普瑞巴林相比,超声引导下高位胸神经节射频治疗带状疱疹后神经痛可以快速有效地减轻患者痛觉,提高长期疗效,且安全性高。

Objective:To evaluate the clinical efficacy and safety of ultrasound-guided high thoracic ganglion radiofrequency therapy for postherpetic neuralgia.Methods:A total of eighty-five patients with postherpetic neuralgia diagnosed and treated in Beijing Shunyi District Hospital from January 2021 to January 2023 were selected as the study objects and divided into control group and treatment group according to random number table method.Control group(42 cases)were treated with pregabalin capsules.Treatment group(43 cases)were treated with radiofrequency therapy of ultrasound-guided high thoracic ganglion radiofrequency on the treatment method of the control group.Visual analogue score(VAS),total effective rate and adverse reactions were compared between the two groups before treatment,1 week,1 month and 3 months after treatment.Results:Before treatment,there was no significant difference in VAS scores between the two groups(P>0.05);after 1 week,1 month and 3 months of treatment,the VAS score of the two groups was lower than before treatment,and that of the treatment group was lower than that of the control group,with statistical significance(P<0.05);the total effective rate of treatment group(86.05%)was higher than that of control group(66.67%),and the difference was statistically significant(P<0.05);there was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Compared with pregabalin,ultrasound-guided high thoracic ganglion radiofrequency therapy for postherpetic neuralgia can reduce pain quickly and effectively,improve long-term efficacy,and have high safety.
