马来酸麦角新碱联合卡贝缩宫素治疗瘢痕子宫妊娠剖宫产术后产妇出血的效果

The Effect of Combination of Ergometrine Maleate and Oxytocin on Postpartum Hemorrhage After Cesarean Section in Scarred Uterine Pregnancy

目的探讨马来酸麦角新碱联合卡贝缩宫素治疗瘢痕子宫妊娠剖宫产术后产妇出血的效果。方法本次探究对象从2022年6月至2023年6月进行剖宫产手术的瘢痕子宫妊娠产妇中选择140例,按照临床药物选择方案差异分为两组,其中将选择卡贝缩宫素治疗的70例患者作为对照组,将马来酸麦角新碱与卡贝缩宫素联合治疗的70例患者作为观察组,记录研究数据后分析两组不同时间段出血情况、凝血指标、康复指标、及不同时间段子宫收缩强度及持续时长情况。结果临床治疗后,观察组产后2 h及24 h内出血量较对照组明显降低,产后出血率低于对照组(P<0.05);干预后,观察组凝血酶时间(TT)、血浆凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)等指标水平较低,血浆纤维蛋白原(FIB)水平明显升高(P<0.05);对比两组治疗后术后排气时间、下床活动时间以及总体住院时间,观察组患者各指标时间较对照组均缩短(均P<0.05);相比于对照组,观察组治疗后不同时间段子宫收缩强度较对照组明显升高,宫缩持续时间下降(P<0.05)。结论选择马来酸麦角新碱与卡贝缩宫素联合治疗进行剖宫产手术的瘢痕子宫妊娠产妇可降低产后出血量,改善凝血功能,促进患者恢复。

Objective Exploring the effect of the combination of ergometrine maleate and oxytocin in the treatment of postpartum hemorrhage after cesarean section in scarred uterine pregnancy.Methods A total of 140 pregnant women with scarred uterus who underwent cesarean section surgery from June 2022 to June 2023 were selected as the research subjects.They were divided into two groups according to the differences in clinical drug selection plans.Among them,70 patients who received treatment with cabetoxin were selected as the control group,and 70 patients who received combined treatment with ergometrine maleate and cabetoxin were selected as the observation group.After recording the research data,the bleeding situation,coagulation indicators,rehabilitation indicators,and uterine contraction intensity and duration at different time periods of the two groups were analyzed.Results After clinical treatment,the bleeding volume within 2 and 24 hours postpartum in the observation group was significantly reduced compared to the control group,and the postpartum bleeding rate was lower than that in the control group(P<0.05);post-intervention,the observation group had lower levels of indicators such as thrombin time(TT),plasma prothrombin time(PT),and activated partial thromboplastin time(APTT),while plasma fibrinogen(FIB)levels were significantly increased(P<0.05);Compared with the control group,the observation group had shorter postoperative exhaust time,bed activity time,and overall hospital stay(P<0.05);Compared with the control group,the intensity of uterine contractions in the observation group significantly increased at different time points after treatment,while the duration of uterine contractions decreased(P<0.05).Conclusions Choosing the combination therapy of ergometrine maleate and cabetoxin for cesarean section can reduce postpartum hemorrhage,improve coagulation function,and promote patient recovery in pregnant women with scarred uterus.
