磁共振弥散加权成像联合磁敏感加权成像检查在急性期脑出血诊断中的效能

Efficiency of magnetic resonance diffusion weighted imaging combined with magnetic susceptibility weighted imaging in diagnosis of acute cerebral hemorrhage

摘要 目的:观察磁共振弥散加权成像(DWI)联合磁敏感加权成像(SWI)检查在急性期脑出血诊断中的效能。方法:选取2021年1月至2023年1月该院收治的99例疑似急性期脑出血患者为研究对象,均进行DWI、SWI序列扫描,以手术结果为金标准,比较DWI、SWI单项及联合检查在急性期脑出血诊断中的效能及其影像学特征。结果:99例疑似急性期脑出血患者中,手术结果确诊阳性48例,阴性51例;DWI检查确诊阳性27例,阴性72例;SWI检查确诊阳性44例,阴性55例;DWI联合SWI检查确诊阳性48例,阴性51例;DWI联合SWI检查诊断急性期脑出血的灵敏度、准确度、阴性预测值均高于DWI、SWI单项检查,差异有统计学意义(P<0.05);DWI、SWI检查急性期脑出血患者均表现为血肿中心极低信号,周围高信号,边缘较清的影像学特征。结论:DWI联合SWI检查诊断急性期脑出血的效能高于二者单项检查。 Objective:To observe efficiency of magnetic resonance diffusion weighted imaging(DWI)combined with magnetic susceptibility weighted imaging(SWI)in diagnosis of acute cerebral hemorrhage.Methods:99 patients with suspected acute cerebral hemorrhage admitted to the hospital from January 2021 to January 2023 were selected as the study subjects.All patients underwent DWI and SWI sequence scanning.Using the surgical results as the gold standard,the efficiencies and imaging features of single and combined examinations of DWI and SWI in diagnosis of acute cerebral hemorrhage were compared.Results:In the 99 patients with suspected acute cerebral hemorrhage,the surgical results showed that 48 cases were positive and 51 cases were negative;DWI examination confirmed 27 positive cases and 72 negative cases;SWI examination confirmed 44 positive cases and 55 negative cases;and DWI combined with SWI confirmed 48 positive cases and 51 negative cases.The sensitivity,the accuracy and the negative predictive value of DWI combined with SWI in the diagnosis of acute cerebral hemorrhage were higher than those of single DWI and single SWI,and the differences were statistically significant(P<0.05).DWI and SWI in the examination of the patients with acute cerebral hemorrhage showed very low signal in the center of the hematoma,high signal around the hematoma,and clear edge.Conclusions:The efficiency of DWI combined with SWI in the diagnosis of acute cerebral hemorrhage is higher than that of the two single examinations.
