SMILE与FS-LASIK治疗屈光性近视患者的效果比较

Comparison of effects of SMILE and FS-LASIK in treatment of patients with refractive myopia

目的:比较飞秒激光小切口角膜基质透镜取出术(SMILE)与飞秒激光辅助准分子激光原位角膜磨镶术(FS-LASIK)治疗屈光性近视患者的效果。方法:回顾性分析2021年6月至2023年7月该院收治的106例屈光性近视患者的临床资料,按照手术方法不同将其分为对照组和观察组各53例。对照组采用FS-LASIK治疗,观察组采用SMILE治疗。比较两组临床疗效,手术前后眼科指标(裸眼视力、眼压)水平、等效球镜度数,以及并发症发生率。结果:两组治疗总有效率和并发症发生率比较,差异均无统计学意义(P>0.05);术后7 d、1个月,两组眼压水平均低于术前,但观察组术后7 d高于对照组,两组裸眼视力水平均高于术前,且观察组术后7 d高于对照组,差异有统计学意义(P<0.05);术后3、7 d,两组等效球镜度数均低于术前,且术后3 d观察组低于对照组,差异有统计学意义(P<0.05)。结论:SMILE治疗屈光性近视患者可改善术后早期眼压、裸眼视力、等效球镜度数,效果优于FS-LASIK治疗,但二者治疗总有效率和并发症发生率相当。

Objective:To compare effects of small incision lenticule extraction(SMILE)and femtosecond laser in situ keratomileusis(FSLASIK)in treatment of patients with refractive myopia.Methods:The clinical data of 106 patients with refractive myopia admitted to this hospital from June 2021 to July 2023 were retrospectively analyzed.According to different surgical methods,they were divided into control group and observation group,53 cases in each group.The control group was treated with FS-LASIK,while the observation group was treated with SMILE.The clinical efficacy,the ophthalmic indexes(naked visual acuity,intraocular pressure)levels,the spherical equivalent refraction level,and the incidence of complications were compared between the two groups before and after the surgery.Results:There were no significant differences in the total effective rate and the incidence of complications between the two groups(P>0.05).7 days and 1 month after the surgery,the intraocular pressure levels of the two groups were lower than those before the surgery,but that in the observation group was higher than that in the control group 7 days after the surgery;the naked visual acuity of the two groups were higher than those before the surgery,and that of the observation group was higher than that of the control group 7 days after the surgery;and the differences were statistically significant(P<0.05).Further,3 and 7 days after the surgery,the spherical equivalent refraction levels of the two groups were lower than those before the surgery,that in the observation group was lower than that in the control group the 3 days after the surgery,and the differences were statistically significant(P<0.05).Conclusions:SMILE can improve the early postoperative intraocular pressure,the naked visual acuity and the spherical equivalent refraction in the patients with refractive myopia.It is superior to FS-LASIK treatment,but the total effective rate and the incidence of complications of the two treatments are equivalent.
