平肝活血方对H型高血压伴左室肥厚病人Hcy、IL-6及心室重构指标的影响

Effect of Pinggan Huoxue Formula on Hcy,IL-6 and Ventricular Remodeling Indexes in H-type Hypertension Patients with Left Ventricular Hypertrophy

目的:观察平肝活血方对H型高血压伴左室肥厚病人同型半胱氨酸(Hcy)、白细胞介素-6(IL-6)及心室重构指标的影响。方法:选取2021年9月—2023年2月河北中医药大学第一附属医院心血管科收治的H型高血压伴左室肥厚病人120例,按随机数字表法分为对照组和中药组,各60例。对照组口服西药沙库巴曲缬沙坦钠片与叶酸片治疗,中药组在口服西药基础上加服中药。治疗12周后,观察并比较两组临床证候疗效、降压疗效、左室舒张末期内径(LVEDD)、室间隔厚度(IVST)、左室后壁厚度(LVPWT)、血清Hcy、IL-6水平。结果:治疗后,中药组中医证候总有效率为95.0%,对照组为76.7%,两组比较差异有统计学意义(P<0.05)。中药组降压疗效总有效率为96.67%,对照组为88.33%,两组比较差异有统计学意义(P<0.05)。治疗后,两组中医证候积分、LVEDD、IVST、LVPWT、Hcy、IL-6均较治疗前降低,且中药组低于对照组,差异均有统计学意义(P<0.05)。结论:平肝活血方可显著降低H型高血压伴左室肥厚病人24 h平均收缩压、24 h平均舒张压,降低血清Hcy、IL-6水平及中医证候积分,改善左室肥厚及临床症状。

Objective:To observe the effects of Pinggan Huoxue formula on homocysteine(Hcy),interleukin-6(IL-6)and ventricular remodeling indexes in H-type hypertension patients with left ventricular hypertrophy.Methods:One hundred and twenty patients with H-type hypertension and left ventricular hypertrophy were divided into control group and Chinese medicine group according to random number table method,with 60 patients in each group.The patients of control group were treated with sacubactril sodium tablets and folic acid tablets,and the patients of Chinese medicine group were treated with Chinese medicine on the basis of oral Western medicine.After 12 weeks of treatment,the clinical syndromic efficacy,antihypertensive efficacy,left ventricular end-diastolic diameter(LVEDD),ventricular septal thickness(IVST),left ventricular posterior wall thickness(LVPWT),serum Hcy and IL-6 levels were compared between the two groups.Results:After treatment,the total effective rate was 95.0% in the Chinese medicine group and that was 76.7% in the control group,with statistical significance(P<0.05).The total effective rate of antihypertensive effect was 96.67% in the Chinese medicine group and that was 88.33% in the control group,with statistical significance(P<0.05).After treatment,traditional Chinese medicine syndrome score,LVEDD,IVST,LVPWT,Hcy and IL-6 in both groups decreased than those before treatment,and those in the Chinese medicine group were less than those in the control group,with statistical significance(P<0.05).Conclusion:Pinggan Huoxue formula could significantly reduce 24 h mean systolic blood pressure,24 h mean diastolic blood pressure,serum Hcy,IL-6,traditional Chinese medicine syndrome score,and improve left ventricular hypertrophy and clinical symptoms in H-type hypertension patients with left ventricular hypertrophy.
