盐酸氨溴索治疗老年慢性阻塞性肺疾病合并肺部感染的临床效果

Clinical Effect of Ambroxol Hydrochloride in the Treatment of Elderly Patients with Chronic Obstructive Pulmonary Disease Complicated with Pulmonary Infection

目的:探讨盐酸氨溴索治疗老年慢性阻塞性肺疾病(简称慢阻肺)合并肺部感染的效果。方法:选取2021年1月—2022年10月盘州市人民医院收治的老年慢阻肺合并肺部感染患者110例作为研究对象,随机分为对照组和研究组,各55例。对照组给予常规对症治疗联合静脉滴注氨茶碱治疗,研究组在对照组基础上联合盐酸氨溴索治疗。比较两组临床症状改善时间及住院时间、肺功能指标、炎性因子水平。结果:研究组咳嗽改善时间、胸闷气短好转时间、肺部湿啰音减弱时间及住院时间均短于对照组,差异有统计学意义(P<0.05);治疗后,两组第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC水平高于治疗前,研究组高于对照组,差异有统计学意义(P<0.05);治疗后,两组血清降钙素原、白细胞介素-6、肿瘤坏死因子-α水平低于治疗前,研究组低于对照组,差异有统计学意义(P<0.05)。结论:在老年慢阻肺合并肺部感染患者治疗中,应用盐酸氨溴索具有显著疗效,可以改善患者的临床症状,提高肺功能,降低炎性因子水平。

Objective:To investigate the effect of ambroxol hydrochloride in the treatment of chronic obstructive pulmonary disease(COPD)complicated with pulmonary infection in the elderly.Methods:A total of one hundred and ten elderly patients with COPD complicated with pulmonary infection admitted to Panzhou People's Hospital from January 2021 to October 2022 were selected as the study objects and randomly divided into control group and study group,with fifty-five cases in each group.The control group was given conventional symptomatic treatment combined with intravenous infusion of aminophylline,and the study group was combined with ambroxol hydrochloride based on the control group.The improvement time of clinical symptoms,hospital stay,lung function index and inflammatory factors were compared between the two groups.Results:The improvement time of cough,the improvement time of short chest stuffy breath,the reduction time of lung moist rales and the length of hospital stay in the study group were shorter than those in the control group,and the difference was statistically significant(P<0.05);after treatment,the levels of forced expiratory volume(FEV_(1)),forced vital capacity(FVC)and FEV_(1)/FVC in the first second of the two groups were higher than before treatment,and the study group was higher than the control group,the difference was statistically significant(P<0.05);after treatment,serum levels of procalcitonin,interleukin-6 and tumor necrosis factor-αin two groups were lower than before treatment,and those in study group were lower than those in control group,the difference was statistically significant(P<0.05).Conclusion:In the treatment of elderly patients with COPD complicated with pulmonary infection,the application of ambroxol hydrochloride has a significant effect,which can improve the clinical symptoms of patients,improve lung function and reduce the level of inflammatory factors.
