非器质性睡眠障碍患者工作记忆与认知损伤相关性

The correlation of working memory and cognitive function in patients with nonorganic insomnia

摘要 目的探讨非器质性睡眠障碍(Nonorganic insomnia,NI)患者与健康对照组(Health controls,HC)的工作记忆与认知功能的相关性。方法本研究纳入50例NI患者及50例HC被试;采用N-back任务进行评估工作记忆;采用词汇流畅性、数字符号替换、连线测试、Stroop色词测试评估两组认知功能,比较NI组与HC组认知功能差异,并探讨工作记忆与认知功能相关性。结果NI组[(1520.18±110.23)ms;(2255.15±409.88)ms]在2-back和3-back任务反应时大于HC组[(1376.10±165.91)ms;(1873.39±208.07)ms],均差异有统计学意义(P<0.05);相较于HC组[(12.68±3.16)s,(38.36±5.23)分,(99.68±22.04)s,(5.28±1.38)s],NI组[(11.44±2.73)s,(35.40±6.63)分,(109.14±24.56)s,(9.56±1.31)s]在词汇流畅性、数字符号替换、连线测试-B、数字符号替换、Stroop抗干扰反应时较差,均差异有统计学意义(P<0.05),NI组3-back测试反应时与数字符号替换呈负相关(r=-0.363,P=0.010),与连线测试A和B呈正相关(r=0.379,P=0.007;r=0.314,P=0.027)。结论NI患者存在工作记忆和认知功能的异常,工作记忆损伤与认知功能密切相关。 Objective To explore the correlation of working memory and cognitive function between nonorganic insomnia(NI)patients and health controls(HC).Methods A total of 50 NI patients were compared with matched 50 HC.Working memory of all the subjects were assessed by N-back test.Cognitive function of all the subjects were assessed by verbal fluency、digital symbol substitution、Trail Making Test、Stroop color word testing,Comparison the correlation between working memory and cognitive function in NI group.Results The reaction time of 2-back and 3-back test in NI group[(1520.18±110.23)ms;(2255.15±409.88)ms]were higher than HC group[(1376.10±165.91)ms;(1873.39±208.07)ms](All,P<0.05).Compared with HC group[(12.68±3.16)s,(38.36±5.23)score,(99.68±22.04)s,(5.28±1.38)s],NI group[(11.44±2.73)s,(35.40±6.63)score,(109.14±24.56)s,(9.56±1.31)s]showed significantly impaired in performance of verbal fluency、digital symbol substitution、Trail Making Test-B、antiinterference reaction time of Stroop task(All,P<0.05).The reaction time of 3-back test in NI group was negatively correlated with Digital symbol substitution in NI group(r=-0.363,P=0.010),but positively correlated with Trail Making Test-A and B(r=0.379,P=0.007;r=0.314,P=0.027).Conclusion The studies find working memory and cognitive function in early NI was impaired,and cognitive function impairment is closely related to working memory.
