泉州地区肺癌患者中医证素特点临床研究

Clinical Research in Traditional Chinese Medicine Syndrome Elements Characteristics of Lung Cancer Patients in Quanzhou

目的探索泉州地区肺癌患者的中医证素分布特点。方法选取福建医科大学附属第二医院收治的肺癌患者130例,进行证素辨证,分析泉州地区的肺癌证素分布特点。结果130例肺癌患者的病性证素分布从高到低依次为阴虚、痰、热、气虚、阳虚、湿、血瘀、血虚;病位证素分布从高到低依次为肺、肾、脾。气虚、阳虚病性证素在60岁及以上人群中占比较高,肺、肾病位证素在≥70岁人群中占比较高;血虚、阴虚、痰及脾证素在女性中占比较高,血瘀证素在男性中占比较高。结论肺癌患者病性证素分布以阴虚、痰、热最为突出,阴虚为肺癌发病的重要中医病理因素;病位证素以肺、脾、肾为主,尤以肺最为突出。在治疗方面,60岁及以上人群需兼顾固护正气和阳气,≥70岁人群需兼顾保肺护肾;女性需兼顾护脾、化痰、固阴血,男性需兼顾活血化瘀;泉州地区肺癌患者应不忘祛湿、化湿。

Objective To explore distribution characteristics of lung cancer syndrome elements in Quanzhou.Methods A total of 130 patients with lung cancer admitted to The Second Affiliated Hospital of Fujian Medical University were selected to analyze the distri-bution characteristics of lung cancer syndromes in Quanzhou.Results The data of syndrome elements(nature of disease)of the 130 cases ranking from high to low are as follow:deficiency of yin,phlegm,heat,deficiency of vital energy,deficiency of yang,humidity,blood stasis,deficiency of blood.The data of syndrome elements(location of disease)ranking from high to low are as follow:lung,spleen,kidney.The proportion of deficiency of vital energy and deficiency of yang is higher in patients above 60 years old and above.The proportion of lung and kidney is higher in patients 70 years old and above.The proportion of deficiency of blood,deficiency of yin,phlegm and spleen is higher in female patients.The proportion of blood stasis is higher in male patients.Conclusion For lung cancer patients,deficiency of yin,phlegm and heat are prominent in distribution of syndrome elements(nature of disease).Lung,spleen,kid-ney are dominant in distribution of syndrome elements(location of disease).Meanwhile,it is discovered that deficiency of yin is a vital TCM pathological factors in the incidence of lung cancer.Patients 60 years old and above need to keep body resistance to diseases and yang-qi,and those 70 years old and above need to protect lung and kidney,female patients need to protect spleen,dissipate phlegm and nourish blood,and male patients need to invigorate blood circulation and eliminate stasis.In the treatment of lung cancer patients in Quanzhou,clearing dampness is important as well.
