改良Widman翻瓣术治疗对重度慢性牙周炎患者牙槽骨状况的影响

Effect of modifiedWidman flap surgery on severe chronic periodontitis and its effect on alveolar bone status

目的探讨改良Widman翻瓣术治疗重度慢性牙周炎的效果及对患者牙槽骨状况的影响。方法选取2020年1月至2022年1月吉安市中心人民医院口腔科收治的96例重度慢性牙周炎患者作为研究对象,根据治疗方案的不同分为对照组与观察组,每组48例。参考组单纯应用牙周基础治疗,观察组在常规应用牙周基础治疗的同时联合应用改良Widman翻瓣术治疗。比较两组临床疗效、牙周指标(牙龈退缩度、龈沟出血指数、牙周袋深度、菌斑指数、附着力丧失)、牙槽骨吸收情况。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗1、2周后,观察组牙龈退缩度、附着力丧失均少于对照组,龈沟出血指数、菌斑指数均低于对照组,牙周袋深度浅于对照组,差异有统计学意义(P<0.05)。治疗1周后,两组牙槽骨吸收情况比较差异无统计学意义;治疗2周后,观察组牙槽骨吸收低于对照组,差异有统计学意义(P<0.05)。结论在常规进行牙周基础治疗的同时联合应用改良Widman翻瓣术治疗重度慢性牙周炎效果显著,可改善患者牙周指标与牙槽骨吸收情况,值得临床推广应用。

Objective To investigate the effect of modified Widman flap surgery on severe chronic periodontitis and its effect on alveolar bone status.Methods A total of 96 patients with severe chronic periodontitis admitted to the Stomatology department of Ji'an Central People's Hospital from January 2020 to January 2022 were selected as the research subjects,and they were divided into the control group and the observation group according to different treatment plans,with 48 cases in each group.The reference group was treated with periodontal basic treatment alone,and the observation group was treated with modified Widman flap surgery combined with conventional periodontal basic treatment.The therapeutic effect,periodontal indexes(gingival receding degree,gingival sulcus bleeding index,periodontal pocket depth,plaque index,adhesion loss)and alveolar bone resorption were compared between the two groups.Results The total effective rate in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).After 1 and 2 weeks of treatment,the gingival receding degree and adhesion loss in the observation group were less than those in the control group,the gingival sulcus bleeding index and plaque index were lower than those in the control group,and the periodontal pocket depth was shallower than that in the control group,the differences were statistically significant(P<0.05).After 1 week of treatment,there was no significant difference in alveolar bone resorption between the two groups;after 2 weeks of treatment,alveolar bone resorption in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The combination of conventional periodontal basic treatment and modified Widman flap surgery in the treatment of severe chronic periodontitis has significant effect,which can improve the periodontal index and alveolar bone absorption of patients,which is worthy of clinical promotion and application.
