运动康复对血液透析患者认知功能及营养状态的影响

摘要 目的探讨运动康复对血液透析患者认知功能及营养状态的影响。方法选取潍坊医学院附属医院和潍坊峡山医院2022年10月至2023年6月收治的120例维持性血液透析(MHD)患者作为研究对象,按抽签法随机分为康复认知训练组(40例)、认知训练组(40例)和对照组(40例)。康复认知训练组在血液透析过程中给予运动康复训练及认知训练。认知训练组给予认知功能训练,不进行运动康复训练。对照组无任何特殊训练。比较三组的蒙特利尔认知量表(MoCA)评分及临床指标。结果干预后,康复认知训练组在视空间执行力、命名、注意、语言、延迟回忆及定向高于干预前及对照组,且视空间执行力、命名、注意、延迟回忆及定向高于认知训练组,差异有统计学意义(P<0.05)。干预后,康复认知训练组患者血红蛋白(HGB)、血浆白蛋白(ALb)、前白蛋白(PA)、体重指数(BMI)高于干预前,且高于认知训练组及对照组,差异有统计学意义(P<0.05)。结论运动康复训练可改善血液透析患者的认知功能及营养状态。 Objective To explore the effect of exercise rehabilitation on cognitive function and nutritional status of hemodialysis patients.Methods A total of 120 patients with maintenance hemodialysis(MHD)admitted to the Affiliated Hospital of Weifang Medical University and Weifang Xiashan Hospital from October 2022 to June 2023 were selected as the study subjects.They were randomly divided into rehabilitation cognitive training group(40 cases),cognitive training group(40 cases)and control group(40 cases)by drawing lots.The rehabilitation cognitive training group was given exercise rehabilitation training and cognitive training during hemodialysis.The cognitive training group was given cognitive function training,but not exercise rehabilitation training.The control group had no special training.The scores of the Montreal cognitive assessment(MoCA)and clinical indicators of the three groups were compared.Results After intervention,the visuospatial executive ability,naming,attention,language,delayed recall and orientation in the rehabilitation cognitive training group were higher than those before intervention and in the control group,and the visuospatial executive ability,naming,attention,delayed recall and orientation were higher than those in the cognitive training group,with statistical significances(P<0.05).After intervention,hemoglobin(HGB),albumin(ALb),prealbumin(PA)and body mass index(BMI)in rehabilitation cognitive training group were higher than before intervention,and higher than cognitive training group and control group,the differences were statistically significant(P<0.05).Conclusion Exercise rehabilitation training can improve the cognitive function and nutritional status of hemodialysis patients.
