皮部引流法治疗腰椎间盘突出症疗效观察

Observation of the clinical effect of skin drainage in the treatment of lumbar disc herniation

目的:观察皮部引流法治疗腰椎间盘突出症(LDH)的临床疗效。方法:将2022年10月—2023年10月在安徽中医药大学第二附属医院脑病三科就诊的确诊为LDH的60例患者随机分为皮部组和针刺组,每组30例。针刺组给予普通针刺治疗,皮部组给予皮部引流法治疗,采用刮痧疗法、刺血疗法联合“工”字形针刺法,2组均5次为1个疗程,共治疗2个疗程。采用视觉模拟评分法(VAS)和日本骨科协会腰椎功能障碍指数(JOA)评估两组患者治疗前后疼痛程度及腰椎功能,比较两组的总体疗效。结果:与治疗前比较,两组患者治疗后VAS评分明显降低,JOA评分明显升高,且皮部组VAS评分降低程度和JOA评分升高程度均高于针刺组,差异具有统计学意义(P<0.05);皮部组临床疗效明显优于针刺组,差异具有统计学意义(P<0.05)。结论:皮部引流法治疗LDH疗效显著。

Objective:To observe the clinical effect of skin drainage in the treatment of lumbar disc herniation(LDH).Methods:A total of 60 patients diagnosed with LDH in the Department III of Encephalopathy of the Second Affiliated Hospital of Anhui University of Chinese Medicine from October 2022 to October 2023 were randomly divided into a skin drainage group and an acupuncture group,with 30 cases in each group.The patients in the acupuncture group were treated with ordinary acupuncture,and the patients in the skin drainage group were treated with skin drainage.Scraping therapy,bloodletting therapy and“Gong”-shaped acupuncture were used.Both groups were treated 5 times as a course of treatment,and a total of 2 courses of treatment were required.The Visual Analogue Score(VAS)and lumbar dysfunction index of Japanese Orthopaedic Association(JOA)were used to evaluate the pain and lumbar function before and after treatment in the two groups,and the overall efficacy of the two groups was compared.Results:Compared with those before treatment,VAS scores were significantly decreased(P<0.05)and JOA scores were significantly increased after treatment in the two groups(P<0.05),and the degrees of VAS score reduction and JOA score increase in the skin drainage group were higher than those in the acupuncture group(P<0.05).The clinical efficacy in the skin drainage group was significantly better than that in the acupuncture group(P<0.05).Conclusion:Skin drainage is effective in the treatment of LDH.
