心房颤动二代冷冻球囊消融术后复发行二次射频消融患者的肺静脉电位特点分析

摘要 目的探讨心房颤动二代冷冻球囊消融术复发行二次射频消融手术患者的肺静脉电位特点。方法既往就诊于泰达国际心血管病医院行二代冷冻球囊消融术治疗的心房颤动患者,选取2017年2月至2023年6月于本院随访中复发,并行二次射频消融术治疗的患者12例。收集患者的一般资料,回顾并收集记录各肺静脉电位恢复特点,比较肺静脉电位恢复与冷冻治疗时60 s内实现肺静脉电位隔离及60 s内达到-40℃情况。结果12例患者复发涉及肺静脉共47支,其中1例患者左侧肺静脉共干,电位恢复的肺静脉16支。其中3支左上肺静脉电位恢复(18.75%),1支左下肺静脉电位恢复(6.25%),3支右上肺静脉电位恢复(18.75%),9支右下肺静脉电位恢复(56.25%)。12例复发患者在二次射频消融时,电隔离肺静脉后,4例增加隔离上腔静脉,1例增加隔离三尖瓣峡部,5例增加三尖瓣峡部线联合上腔静脉消融,1例增加心房扑动消融,1例未增加肺静脉外其他部位消融。术后随访心房扑动患者术后再发心房扑动,余患者均能维持窦性心律。二代冷冻球囊消融术中60 s内是否实现肺静脉电位隔离,肺静脉电位恢复差异无统计学意义(χ^(2)=3.116,P>0.05),60 s内是否达到-40℃,肺静脉电位恢复差异有统计学意义(χ^(2)=3.948,P<0.05),60 s内是否实现肺静脉电位隔离并达到-40℃,肺静脉电位恢复差异有统计学意义(χ^(2)=3.923,P<0.05)。结论心房颤动行二代冷冻球囊消融术后复发患者肺静脉电位恢复多位于右下肺静脉,术中实现60 s内肺静脉电位隔离同时达到-40℃可提高手术效果,无法记录到肺静脉电位隔离时,温度可作为肺静脉隔离的有效评估指标。
