MRI多模态技术对鞍区肿瘤的应用价值分析

摘要 目的:探讨MRI多模态技术对鞍区肿瘤的应用价值。方法:回顾性收集2020年1月—2023年12月西南医科大学附属中医医院临床诊治的117例鞍区肿瘤患者的资料进行研究,包括脑膜瘤25例、胶质瘤11例、垂体大腺瘤21例、垂体微腺瘤27例、颅咽管瘤19例、转移瘤14例。收集所有病例治疗前首次头颅MRI平扫(T_(1)WI、T_(2)WI、FLAIR、DWI)及增强扫描资料,分析MRI主要征象或典型征象经多模态扫描的显示情况。结果:本组鞍区肿瘤主要征象有:脑膜尾征、瘤囊混合征、鞍底下陷征、跨鞍隔征、均匀强化征、不均匀强化征、壳样强化征、低强化征。其中,脑膜尾征26例,常规增强均可显示;瘤囊混合征45例,平扫序列及增强扫描均有显示,增强扫描显示率50.00%;鞍底下陷征33例、跨鞍隔征16例,平扫及增强扫描矢状位、冠状位显示较好;均匀强化征51例、不均匀强化征63例,常规增强与动态增强均有较高的显示率;壳样强化征14例,常规增强均能显示;低强化征26例,动态增强显示率96.30%。结论:MRI多模态技术以及合理的扫描序列和方位,能够很好显示鞍区肿瘤的影像征象,从而为临床诊断和治疗提供可靠的依据。 Objective This study aims to explore the application value of MRI multimodal technology in the diagnosis of sellar tumors.Methods Retrospective collection of data from 117 patients with sellar region tumors treated clinically at Affiliated Traditional Chinese Medicine Hospital of Southwest Medical University from January 2020 to December 2023 were selected for research,including 25 cases of meningiomas,11 gliomas,21 pituitary macroadenomas,27 pituitary microadenomas,19 craniopharyngiomas,and 14 metastatic tumors.The pre-treatment skull MRI data for all cases,including plain scans(T_(1)WI,T_(2)WI,FLAIR,DWI)and enhanced scans,were collected.Among them,the main or typical signs displayed by multimodal scanning MRI were analyzed.Results The main signs of sellar tumors in this group included:dural tail sign,tumor cyst mixed sign,sellar floor sinking sign,trans-sellar septum sign,uniform enhancement sign,uneven enhancement sign,shell-like enhancement sign,and low enhancement sign.Among them,the dural tail sign was observed in 26 cases and could be displayed by conventional enhancement;the tumor cyst mixed sign was observed in 45 cases,with both plain and enhanced scans showing this sign,and an enhanced scan display rate of 50.00%;sellar floor sinking sign in 33 cases and trans-sellar septum sign in 16 cases were better displayed in sagittal and coronal positions with plain and enhanced scans;uniform enhancement sign in 51 cases and uneven enhancement sign in 63 cases had a high display rate in both conventional and dynamic enhancement;shell-like enhancement sign was observed in 14 cases and could be displayed by conventional enhancement;low enhancement sign was observed in 26 cases,with a dynamic enhancement display rate of 96.30%.Conclusion MRI multimodal technology,along with reasonable scanning sequences and orientations,can effectively display the imaging signs of sellar tumors,thereby providing a reliable basis for clinical diagnosis and treatment.
