溃疡性结肠炎和克罗恩病患者EB病毒抗体谱及预后分析

EB virus antibody profile and prognosis of patients with ulcerative colitis and Crohn′s disease

目的探讨溃疡性结肠炎(ulcerative colitis,UC)和克罗恩病(Crohn’s disease,CD)患者EB病毒抗体谱并分析预后。方法回顾性分析首都医科大学附属北京友谊医院2015年1月至2021年6月95例炎症性肠病患者,其中UC患者64例,CD患者31例。所有患者均进行EB病毒抗体检测,包括EBV-CA-IgM、EBV-CA-IgG、EBV-EA-IgG、EBV-NA-IgG四项抗体,并开展1年随访观察预后。结果CD组EBV-CA-IgM阳性率为41.94%,明显高于UC组的18.75%(P<0.05)。在EB病毒抗体谱[EBV-CA-IgM(+)、EBV-CA-IgG(+)、EBV-EA-IgG(-)、EBV-NA-IgG(+)]中,CD组的比例(32.26%)高于UC组(12.50%)(P<0.05)。UC组EBV-CA-IgM阳性、阴性患者的预后良好率分别为83.33%、92.31%(P>0.05)。CD组EBV-CA-IgM阳性、阴性患者的预后良好率分别为76.92%、94.44%(P>0.05)。UC组不同EB病毒抗体谱的预后良好率分别为50.00%、100%、87.50%、100%、91.67%、85.71%、100%、100%(P>0.05)。CD组不同EB病毒抗体谱的预后良好率分别为100%、100%、90.00%、100%、84.62%、100%、100%、100%(P>0.05)。结论UC和CD患者EB病毒抗体谱存在差异,CD患者EBV-CA-IgM阳性率更高,不过EB病毒抗体谱与预后无明显关联。

Objective To investigate the Epstein-Barr(EB)virus antibody profile in patients with ulcerative colitis(UC)and Crohn′s disease(CD),and analyze the prognosis.Methods A retrospective study was conducted among 95 patients with inflammatory bowel diseases(64 patients with UC and 31 patients with CD)who were treated in Beijing Friendship Hospital,Capital Medical University from Jan.2015 to Jun.2021.All patients were tested for EB virus antibodies,including EBV-CA-IgM,EBV-CA-IgG,EBV-EA-IgG and EBV-NA-IgG.One-year follow-up was performed to observe the prognosis.Results The positive rate of EBV-CA-IgM in CD group(41.94%)was significantly higher than that in UC group(18.75%)(P<0.05).In the EB virus antibody profile[EBV-CA-IgM(+),EBV-CA-IgG(+),EBV-EA-IgG(-)and EBV-NA-IgG(+)],the proportion in CD group(32.26%)was higher than that in the UC group(12.50%)(P<0.05).The good prognosis rates of EBV-CA-IgM positive patients and negative patients in UC group were 83.33%and 92.31%,respectively(P>0.05).The good prognosis rates of EBV-CA-IgM positive patients and negative patients in CD group were 76.92%and 94.44%,respectively(P>0.05).The good prognosis rates of patients with different EB virus antibody profiles in UC group were 50.00%,100%,87.50%,100%,91.67%,85.71%,100%and 100%,respectively(P>0.05).The good prognosis rates of patients with different EB virus antibody profiles in CD group were 100%,100%,90.00%,100%,84.62%,100%,100%and 100%,respectively(P>0.05).Conclusion Patients with UC and CD have different EB virus antibody profiles.The positive rate of EBV-CA-IgM is higher in patients with CD,but there is no significant correlation between EB virus antibody profile and the prognosis.
