颞部短切口超高位SMAS瓣制备在中面部提升中的临床应用

Clinical Application of Short Temporal Incision Ultra-High SMAS Flap in Midface Lift

目的这项调查以中国患者为样本,评估了患者对采用超高浅表肌肉神经系统(Super-Elevated Superficial Musculoaponeurotic System,SMAS)皮瓣的颞部切口技术进行中面部提升术的满意度。方法2019年7月-2023年7月,81例患者在我院接受了颞部短切口中面部提升术。SMAS皮瓣剥离范围上至眉尾,下至颧弓下缘,内至颧大肌。将分离完的SMAS瓣向外上固定在颞深筋膜,以达到提升中面部颧脂肪垫,进一步改善鼻唇沟的美容效果。结果本组81例患者术后随访6个月~2年,有效随访55例,大多数患者对自己中面部的提升效果满意,未发生面神经颞支永久损伤等严重并发症。结论颞部切口除皱因为切口短,更容易受到患者接受,但是因为术野狭小,对术者要求高,术者必须对面部解剖非常熟悉;另外对于面部松垂过于松垂的,还是建议患者选择长切口手术方式。

Objective This investigation assessed patient satisfaction with a temporal incision technique employing a superelevated superfificial musculoaponeurotic system(SMAS)flflap for midface lifting in a Chinese patient cohort.Methods From July 2019 to July 2023,81 patients underwent midface lifting with a short temporal incision in Jining Xingyang Cosmetic Surgery Hospital.The extent of SMAS flflap dissection spanned from the tail of the eyebrow superiorly,to the lower margin of the zygomatic arch inferiorly,and medially to the zygomaticus major muscle.The technique involved anchoring the elevated SMAS flflap to the deep temporal fascia to elevate the malar fat pads,correct sagging tissues,and ameliorate the appearance of nasolabial folds.Results Among the 81 patients,55 completed a follow-up ranging from 6 months to 2 years post-surgery,with a majority reporting satisfaction with the aesthetic results of their midface lift.No significant complications,including permanent facial nerve injury,were reported.Conclusions Temporal incision facelifts are increasingly preferred by patients for their minimally invasive nature and shorter recovery time.However,the success of this technique hinges on a comprehensive understanding of facial anatomy due to the confifined operative fifield,highlighting its technical demands and the need for surgical expertise.
