光动力疗法治疗痤疮后瘢痕疙瘩的疗效及影响因素分析

摘要 目的探讨光动力疗法治疗痤疮后瘢痕疙瘩的疗效及安全性。方法选取河南大学第一附属医院皮肤科2020年1月-2022年3月收治的痤疮后瘢痕疙瘩患者92例,根据治疗方法将其分为对照组(46例)和观察组(46例)。对照组采用复方倍他米松治疗,观察组在其基础上加用光动力疗法治疗,评估两组患者瘢痕治疗效果及不良反应。结果两组治疗后1~2个月温哥华瘢痕量表评分和北卡罗来纳大学瘢痕量表评分均低于治疗前,治疗后2个月均低于治疗后1个月,且观察组显著低于对照组(P<0.05);两组不良反应发生率无差异(P>0.05);轻中度瘢痕的治疗效果优于重度瘢痕(P<0.05)。结论光动力疗法能够明显改善瘢痕情况,显著提高临床疗效,瘢痕部位是影响疗效的主要因素。 Objective To investigate the effificacy and inflfluencing factors of photodynamic therapy in the treatment of post acne keloid.Method A total of 92 patients with post acne keloid admitted to the Department of Dermatology,the First Affifiliated Hospital of Henan University from January 2020 to March 2022 were selected.They were divided into control group(46 cases)and observation group(46 cases)according to the treatment method.The control group was treated with compound betamethasone,and the observation group was treated with photodynamic therapy on the basis of compound betamethasone.The scar treatment effect and adverse reactions of the two groups were evaluated.Result The Vancouver Scar Scale score and the University of North Carolina scar scale score of the two groups were lower than those before treatment 1-2 months after treatment,and were lower than those 1 month after treatment fi2 months after treatment,and the observation group was significantly lower than that of the control group(P<0.05);There was no difference in the incidence of adverse reactions between the two groups(P>0.05);The treatment effect of mild to moderate scar was better than that of severe scar(P<0.05).Conclusion fiPhotodynamic therapy can significantly improve the scar condition and clinical curative effect,and the scar position is the main factor affecting the curative effect.
