摘要
目的:探讨早期持续气道正压通气(CPAP)在小儿重症肺炎合并呼吸衰竭救治中的应用效果.方法:选取2021年1月至2023年1月本院收治的116例重症肺炎合并呼吸衰竭患儿为研究对象.随机将其分为CPAP组(n=58,给予CPAP治疗)和常规组(n=58,给予经鼻导管吸氧治疗).采用统计学方法,对两组患者动脉血气分析指标[动脉血二氧化碳分压(PaCO_(2))、动脉血氧饱和度(SaO_(2))、动脉血氧分压(PaO_(2))],治疗疗效,临床症状缓解时间和并发症总发生率进行比较.结果:治疗后,CPAP组患儿PaCO_(2)低于常规组,SaO_(2)、PaO_(2)高于常规组,差异均有统计学意义(P<0.05).CPAP组治疗总有效率高于常规组,临床症状缓解时间均短于常规组,并发症总发生率低于常规组,差异均有统计学意义(P<0.05).结论:与经鼻导管吸氧治疗相比较,CPAP在小儿重症肺炎合并呼吸衰竭救治中的临床效果更好,适合临床推广应用.
Objective:To explore the application effect of early continuous positive airway pressure(CPAP)in the treatment of patients with severe pneumonia complicated with respiratory failure in children.Methods:A total of 116 children with severe pneumonia and respiratory failure treated in our hospital from January 2021 to January 2023 were selected as the research objects.And they were randomly divided into CPAP group(n=58,given CPAP treatment)and conventional group(n=58,given nasal catheter oxygen inhalation treatment).Arterial blood gas indicators[arterial partial pressure of carbon dioxide(PaCO_(2)),arterial oxygen saturation(SaO_(2))and arterial partial pressure of oxygen(PaO_(2))],curative effect,improvement of clinical symptoms,and the incidence of complications were compared between the two groups by statistical method.Results:After treatment,PaCO_(2)in CPAP group was lower than that in conventional group,SaO_(2)and PaO_(2)were higher than those in conventional group,the differences were statistically significant(P<0.05).The total curative effective rate of CPAP group was higher than that of conventional group,the clinical symptom relief time of CPAP group was shorter than that of conventional group,and the incidence of complication of CPAP group was lower than that of conventional group,and the differences were statistically significant(P<0.05).Conclusion:Compared with nasal cannula oxygen therapy,CPAP has a better clinical effect in the treatment of severe pneumonia combined with respiratory failure in children,making it more suitable for clinical application.
