男性血液透析患者性功能障碍现状及影响因素

Present situation and influencing factors of sexual dysfunction in male patients undergoing hemodialysis

目的调查男性血液透析患者性功能障碍现状及其影响因素,为提高男性血液透析患者生活质量提供依据。方法选取2022年5月至2023年5月在我院接受规律性、维持性血液透析治疗的102例男性患者作为研究对象,应用一般资料调查表、汉密尔顿抑郁量表、国际勃起功能指数-5量表为调查工具进行问卷调查,采用单因素分析、有序Logistic回归分析男性血液透析患者勃起功能指数的影响因素。结果勃起功能正常患者7例,轻度障碍25例,中度障碍16例,重度障碍54例;对不同程度勃起障碍患者年龄、血清肌酐、血清尿酸、抑郁指数等进行对比,有统计学差异(P<0.05);有序Logistic回归分析结果显示,抑郁指数(OR=1.39)、年龄(OR=1.14)、文化程度(OR=7.29)、睡眠时间(OR=32.84)、工作状态(OR=0.08)是男性血液透析患者勃起功能障碍的影响因素(P<0.05)。结论男性血液透析患者勃起功能障碍发生率较高,且以中重度为多。抑郁、年龄、文化程度、睡眠时间及工作状态为勃起功能障碍的独立影响因素,临床应针对抑郁、年龄、文化程度、睡眠时间等因素采取相关措施,以降低其发生率,提高患者生活质量。

Objective To investigate present situation and influencing factors of sexual dysfunction in male hemodialysis patients,and obtain evidence for improving the quality of life of male hemodialysis patients.Methods A total of 102 male patients receiving regular maintenance hemodialysis treatment in our hospital from May 2022 to May 2023 were selected as the study objects,and the General Information Questionnaire,Hamilton Depression Scale and International Erectile Function Index 5 were applied as the investigation tools.Univariate analysis and ordered Logistic regression were used to analyze the influencing factors of erectile function index in male hemodialysis patients.Results There were 7 patients with normal erectile function,25 with mild dysfunction,16 with moderate dysfunction and 54 with severe dysfunction.Analysis of age,serum creatinine,serum uric acid and depression index among the patients with different degrees of erectile dysfunction demonstrated obviously statistical differences(P<0.05).Ordered Logistic regression analysis showed that depression index(OR=1.39),age(OR=1.14),education level(OR=7.29),sleep time(OR=32.84)and working status(OR=0.08)were the influencing factors of erectile dysfunction in male hemodialysis patients(P<0.05).Conclusion The incidence of erectile dysfunction in male hemodialysis patients is higher,and most of them are moderate and severe.Depression,age,education level,sleep time and working status are independent influencing factors of erectile dysfunction.Clinical measures should be taken to reduce the incidence of depression,age,education level,sleep time and other factors to improve the quality of life of patients.
