依托咪酯与丙泊酚在全身麻醉患者中的应用效果及对肾上腺皮质功能的影响比较

Comparison of Effects of Etomidate and Propofol on Adrenal Function in Patients with General Anesthesia

目的:比较依托咪酯与丙泊酚在全身麻醉患者中的应用效果及对肾上腺皮质功能的影响。方法:选取2022年1—12月拟于昆明同仁医院行全身麻醉手术的患者106例为研究对象,采用随机数字表法分为对照组和试验组,各53例。对照组采用丙泊酚麻醉,试验组采用依托咪酯麻醉。比较两组麻醉效果及肾上腺皮质功能。结果:试验组麻醉诱导时间、苏醒时间、拔管时间、麻醉后定向力恢复时间短于对照组,镇静效果优于对照组,差异有统计学意义(P<0.001)。麻醉前,两组醛固酮、皮质醇(Cor)水平比较,差异无统计学意义(P>0.05);术后24h,两组醛固酮、Cor水平降低,试验组低于对照组,差异有统计学意义(P<0.05)。结论:依托咪酯在全麻手术中的麻醉效果更好,且术后短期可抑制患者肾上腺皮质功能。

Objective:To compare the effects of etomidate and propofol on adrenal function in patients with general anesthesia.Methods:A total of one hundred and six patients scheduled to undergo general anesthesia surgery in Kunming Tongren Hospital from January to December 2022 were selected as the study objects,and were divided into control group and experimental group by random number table method,with fifty-three cases in each group.The control group was anesthetized with propofol,and the experimental group was anesthetized with etomidate.The anesthetic effect and adrenal cortex function were compared between the two groups.Results:Anesthesia induction time,recovery time,extubation time and orientation recovery time after anesthesia in experimental group were shorter than those in control group,and sedation effect was better than that in control group,the difference was statistically significant(P<0.001).Before anesthesia,there was no significant difference in the levels of aldosterone and cortisol(Cor)between the two groups(P>0.05);24 h after operation,the levels of aldosterone and Cor in two groups were decreased,and the experimental group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion:Etomidate has better anesthetic effect in general anesthesia,and can inhibit the adrenal cortex function in the short term after operation.
