醋酸亮丙瑞林联合孕三烯酮对子宫内膜异位症术后患者的疗效及免疫炎症水平的影响

Efficacy and impact on immune-inflammatory levels of leuprorelin acetate combined with gestrinone in postoperative patients with endometriosis

目的探究醋酸亮丙瑞林联合孕三烯酮对子宫内膜异位症术后患者的疗效及免疫功能影响。方法纳入2020年1月至2022年6月收治的子宫内膜异位症患者共152例,随机分为观察组和对照组,每组76例。所有患者行全麻下经腹腔镜子宫内膜异位症切除术。术后,对照组予醋酸亮丙瑞林治疗,观察组予亮丙瑞林联合孕三烯酮治疗,治疗6个月后随访。检测2组术前及术后血清中雌二醇(E2)、黄体生成素(LH)、卵泡刺激素(FSH)、白细胞介素2(IL-2)、白细胞介素6(IL-6)及白细胞介素10(IL-10)水平。评估2组疗效、不良事件总发生率及术后6个月治疗费用。结果术后6个月,2组血清E2、LH及FSH水平比较差异无统计学意义(P>0.05);2组IL-4、IL-6水平较术前均显著降低,IL-2水平较治疗前均显著升高(P<0.05),且观察组较对照组变化更明显(P<0.05)。2组总有效率差异无统计学意义(P>0.05)。观察组不良事件总发生率低于对照组(P<0.05)。观察组术后6个月的治疗费用低于对照组(P<0.05)。结论醋酸亮丙瑞林联合孕三烯酮能够改善子宫内膜异位症患者术后的炎症水平,降低不良事件发生率和术后治疗费用,具有较好的临床应用前景。

AIM To investigate the efficacy and impact on immune function of leuprorelin acetate combined with gestrinone in postoperative patients with endometriosis.METHODS From January 2020 to June 2022,152 patients with endometriosis were randomly divided into observation group and control group,with 76 cases in each group.All patients underwent laparoscopic endometriosis resection under general anesthesia.Patients in the control group were treated with leuprorelin injection after operation,while patients in the observation group were treated with leuprorelin combined with gestrinone postoperatively,with a follow-up after 6 months of treatment.The serum levels of estradiol(E2),luteinizing hormone(LH),follicle-stimulating hormone(FSH),interleukin-2(IL-2),interleukin-6(IL-6)and interleukin-10(IL-10)in 2 groups before and after surgery were measured.The efficacy,the incidence of adverse events and the cost of postoperative treatment were evaluated in 2 groups.RESULTS There was no significant difference in serum E2,LH and FSH levels between 2 groups 6 months after surgery(P>0.05).The levels of IL-4 and IL-6 decreased significantly,and the level of IL-2 increased significantly in both groups after surgery compared to before treatment(P<0.05).The changes in the observation group were more obvious than those in the control group(P<0.05).There was no significant difference in the total effective rate between 2 groups(P>0.05).The incidence of adverse events in the observation group was lower than that in the control group(P<0.05),and the treatment cost for 6 months after surgery in the observation group was lower than that in the control group(P<0.05).CONCLUSION Leuprorelin acetate combined with gestrinone can improve postoperative inflammation levels in patients with endometriosis,and reduce the incidence of adverse events and the cost of postoperative treatment,which has a good prospect for clinical application.
