尘肺病患者血清胆红素、前列腺素E_(2)与病情、肺功能的相关性

Correlation of serum bilirubin and prostaglandin E_(2) with disease condition and lung function in patients with pneumoconiosis

摘要 目的 探讨血清总胆红素(TBIL)、前列腺素E_(2)(PGE_(2))与尘肺病患者病情进展和肺功能损伤的关系及其评估价值。方法 纳入2021年8月~2023年8月本院治疗的105例尘肺病患者为尘肺组,依据病情进展分为Ⅰ期组58例和Ⅱ/Ⅲ期组47例,依据肺功能损伤分为轻度组64例和中度/重度组41例;纳入107例体检健康人群为对照组。采用酶联免疫吸附法检测血清TBIL、PGE_(2)水平,采用肺功能检测仪检测肺功能指标第1 s用力呼气容积(FEV_(1))、用力肺活量(FVC)、最大通气量(MVV),计算FEV_(1)/FVC;采用Pearson相关性分析尘肺病患者血清TBIL、PGE_(2)水平与肺功能指标的相关性;采用受试者工作特征(ROC)曲线分析血清TBIL、PGE_(2)水平对尘肺病病情进展及肺功能损伤的评估价值。结果 尘肺病组血清TBIL、PGE_(2)水平高于对照组(P<0.05)。尘肺病患者血清TBIL、PGE_(2)水平与FEV_(1)、FEV_(1)/FVC、MVV均呈负相关(P<0.05)。Ⅱ/Ⅲ期组血清TBIL、PGE_(2)水平高于Ⅰ期组(P<0.05)。血清TBIL、PGE_(2)单独及联合评估尘肺病病情进展的AUC分别为0.748、0.828、0.895。中度/重度组血清TBIL、PGE_(2)水平高于轻度组(P<0.05)。血清TBIL、PGE_(2)单独及联合评估尘肺病肺功能损伤的AUC分别为0.868、0.849、0.943。结论 尘肺病患者血清TBIL、PGE_(2)水平显著升高,且与病情进展及肺功能损伤有关。 Objective To explore the relationship of serum total bilirubin(TBIL)and prostaglandin E_(2)(PGE_(2))with the progression of pneumoconiosis and lung function damage,and their evaluation value.Methods A total of 105 cases of pneumoconiosis treated in our hospital from August 2021 to August 2023 were included as the pneumoconiosis group.According to the progression of the disease,58 cases were grouped into the phaseⅠgroup and 47 cases into the phaseⅡ/Ⅲgroup.64 cases were grouped into the mild group and 41 cases into the moderate/severe group based on lung function damage.107 healthy individuals undergoing physical examination were included as the control group.Enzyme-linked immunosorbent assay was applied to detect serum TBIL and PGE_(2) levels,the lung function detector was applied to detect the forced expiratory volume in first second(FEV_(1)),forced vital capacity(FVC),and maximum ventilation volume(MVV)of lung function indicators,and FEV_(1)/FVC was calculated.Pearson correlation was applied to analyze the correlation between serum TBIL,PGE_(2) levels and lung function indicators in pneumoconiosis patients.Receiver operating characteristic(ROC)curve was applied to analyze the evaluation value of serum TBIL and PGE_(2) levels in the progression of pneumoconiosis and lung function damage.Results The serum TBIL and PGE_(2) levels in the pneumoconiosis group were higher than those in the control group(P<0.05).The serum levels of TBIL and PGE_(2) in pneumoconiosis patients were negatively correlated with FEV_(1),FEV_(1)/FVC,and MVV(P<0.05).The levels of serum TBIL and PGE_(2) in the phaseⅡ/Ⅲgroup were higher than those in the phaseⅠgroup(P<0.05).The AUC of serum TBIL and PGE_(2) for evaluating the progression of pneumoconiosis was 0.748,0.828,and 0.895,respectively,and the levels of serum TBIL and PGE_(2) in the moderate/severe group were higher than those in the mild group(P<0.05).The AUC of serum TBIL and PGE_(2) for evaluating pulmonary function damage in pneumoconiosis was 0.868,0.849,and 0.943,respectively.Conclusion The serum levels of TBIL and PGE_(2) in pneumoconiosis patients are obviously elevated, and are related to the progression of the disease and lung function damage.
