超短效多次胰岛素皮下注射对2型糖尿病患者胰岛β细胞功能和血糖控制的影响

Effect of ultra-short-acting multiple subcutaneous insulin injections on islet beta cell function and blood glucose control in patients with type 2 diabetes

目的:探讨超短效多次胰岛素皮下注射对2型糖尿病(T2DM)患者胰岛β细胞功能和血糖控制的影响。方法:研究对象选取2022年6月至2023年12月门诊收治的60例T2DM患者为研究对象,均给予超短效多次胰岛素皮下注射治疗,观察治疗前后胰岛β细胞功能、血糖控制情况。同时,将60例T2DM患者按糖化血红蛋白(HbA1c)水平分为低HbA1c组32例和高HbA1c组28例,比较两组治疗前后胰岛β细胞功能、血糖变化差值。结果:治疗后,胰岛β细胞功能指数[HOMA-β(C-P)In]、空腹C肽(FC-P)相对于治疗前明显提升(P<0.05),胰岛素抵抗指数[HOMA-IR(C-P)In]、空腹血糖(FBG)相对于治疗前均明显降低(P<0.05);低HbA1c组患者治疗前后HOMA-β(C-P)In差值、HOMA-IR(C-P)In差值、FBG差值高于高HbA1c组(P<0.05),但两组FC-P差值无统计学意义(P>0.05)。结论:超短效多次胰岛素皮下注射治疗可改善T2DM患者胰岛β细胞,有效控制血糖,且低HbA1c患者治疗前后血糖、胰岛素β细胞功能改善相对更佳。

Objective:To investigate the effects of ultra-short-acting multiple subcutaneous insulin injections on islet beta cell function and blood glucose control in patients with type 2 diabetes mellitus(T2DM).Methods:60 T2DM patients admitted to the outpatient department from June 2022 to December 2023 were selected as the study subjects,all of whom were given ultra-short-action multiple subcutaneous insulin injections,and the function of islet beta cells and blood glucose control before and after treatment were observed.At the same time,60 T2DM patients were divided into low HbA1c group(32 cases)and high HbA1c group(28 cases)according to HbA1c level,and the difference of isletβcell function and blood glucose before and after treatment was compared between the two groups.Results:After treatment,isletβcell function index[HOMA-β(C-P)In]and fasting C-peptide(FC-P)were significantly increased compared with before treatment(P<0.05),insulin resistance index[HOMA-IR(C-P)In]and fasting blood glucose(FBG)were significantly decreased compared with before treatment(P<0.05).The difference of HOMA-β(C-P)In、HOMA-IR(C-P)In and FBG before and after treatment in low HbA1c group was higher than that in high HbA1c group(P<0.05),but the difference of FC-P between the two groups was not statistically significant(P>0.05).Conclusion:Ultra-short-acting multiple subcutaneous insulin injection can improve islet beta cells and effectively control blood glucose in T2DM patients,and blood glucose and insulin beta cell function are relatively better in patients with low HbA1c before and after treatment.
