胸部CT上腔静脉定位与体外测量公式在非小细胞肺癌患者PICC置管长度测量中的应用

摘要 目的比较胸部CT上腔静脉定位与体外测量公式在非小细胞肺癌(non-small cell lung cancer,NSCLC)患者,在经外周静脉穿刺置入中心导管(peripherally inserted central venous catheter,PICC)长度测量中的应用效果。方法回顾性分析在杭州市拱墅区天水武林街道社区卫生服务中心接受PICC置管的82例NSCLC患者的临床资料。根据PICC置管长度测定方式将患者分为观察组和对照组各41例。观察组采用胸部CT上腔静脉定位测定;对照组采用体外测量公式进行计算,比较两组患者PICC置管一次到位率、置管操作时间、留置时间、疼痛评分、机械性静脉炎发生情况及护理满意度。结果两组PICC置管操作时间、疼痛评分差异无统计学意义(P>0.05);观察组PICC置管一次到位率、PICC置管留置时间、穿刺部位机械性静脉炎发生情况、护理满意度均优于对照组,差异有统计学意义(P<0.05)。结论胸部CT上腔静脉定位法有利于提升PICC置管一次到位率,延长留置时间,降低机械性静脉炎发生风险和提高护理满意度。 Objective To compare the effect of chest CT vena cava localization and in vitro measurement formula in measuring the length of peripherally inserted central venous catheter(PICC)in patients with non-small cell lung cancer(NSCLC).Methods The clinical data of 82 patients with NSCLC who received PICC catheterization in Community Health Service Center of Tianshui Wulin Street of Gongshu District were retrospectively analyzed.The patients were divided into an observation group and a control group,with 41 cases in each group,according to the PICC catheterization length measurement method.The observation group was determined by chest CT superior vena cava location,while the control group was calculated by in vitro measurement formula.The rate of PICC placement once,time of catheterization,time of retention,pain score,occurrence of mechanical phlebitis and nursing satisfaction were compared between the two groups.Results There was no significant difference in PICC operation time and pain score between the two groups(P>0.05).The correct place rate of PICC catheterization,retention time of PICC catheterization,occurrence of mechanical phlebitis at puncture site and nursing satisfaction of patients in observation group were significantly better than those in control group(P<0.05).Conclusion Thoracic CT superior vena cava localization method is beneficial to improve the PICC catheterization rate,prolong the indenture time,reduce the risk of mechanical phlebitis,and improve nursing satisfaction.
