白芥子涂方对哮喘治疗中的免疫调节作用研究

摘要 目的:探讨白芥子涂方在哮喘治疗中的免疫调剂作用机制。方法:构建SD大鼠哮喘模型,随机分为正常对照组、模型组、白芥子涂方敷贴非穴位组、白芥子涂方敷贴肺俞穴组、阳性对照组(氨茶碱敷贴肺俞穴),给药14 d,收集血清、肺、脾、胸腺、肝脏及肾脏组织。评估淋巴细胞增殖及巨噬细胞活化,检测血清细胞因子水平,分析器官指数、形态学及皮肤树突状细胞(dendritic cells,CD)细胞数,检测CD90及H2-Ab1表达改变。结果:与模型组相比,白芥子涂方敷贴穴位组及氨茶碱敷贴可以诱导T淋巴细胞增殖(P<0.01),增加IgA、IgG、IgM、IFN-γ及IL-2(P<0.01),但降低IL-4及MBP水平(P<0.01)。此外,白芥子涂方敷贴肺俞穴组贴及氨茶碱敷贴肺俞穴组皮肤组织中淋巴细胞浸润减少,细胞肿胀数降低,支气管管腔内黏液分泌降低,气道水肿及闭塞减少,肺间质炎性浸润伴充血情况减少。皮肤组织中CD1a^(+)CD83^(-)未成熟DC细胞减少,而CD1a^(+)CD83^(+)成熟DC细胞增加,皮肤组织中CD90及H2-Ab1表达显著性增加。结论:白芥子涂方穴位敷贴治疗可促进皮肤组织中髓系树突状细胞发生形变、迁移,并促进抗原的呈递作用,可以激活机体免疫功能,且对机体不良反应低,能发挥调节机体Th1/Th2的失衡的作用。 OBJECTIVE To explore the immunomodulatory effect of Baijiezi Tufang Decoction(BTD)on treating asthma.METHODS SD rat asthma model was constructed.And the animals were randomized into five groups of control,model,BTD applied to non-acupoint,BTD applied to Feishu acupoint and positive control(aminophylline applied to Feishu acupoint).Serum samples,lung,spleen,thymus,liver and kidney tissues were harvested after 14-day dosing.Lymphocyte proliferation and macrophage activation were evaluated,serum levels of cytokines quantified,organ indices,morphology and skin DC cell count examined and expression changes of CD90 and H2-Ab1 detected.RESULTS As compared with model group,acupoint application of BTD and aminophylline could induce T lymphocyte proliferation(P<0.01),leading to the elevations of IgA,IgG,IgM,IFN-γand IL-2(P<0.01)and yet the reductions of IL-4 and MBP(P<0.01).In addition,lymphocyte infiltration,cell swelling number,mucus secretion in bronchial lumen,airway edema and occlusion,pulmonary interstitial inflammatory infiltration and congestion improved in skin tissue of Feishu acupoint and aminophylline groups.Further study indicated that CD1a^(+)CD83^(-)immature DC cells dropped while CD1a^(+)CD83^(+)mature DC cells spiked and the expressions of CD90 and H2-Ab1 became markedly upregulated in skin tissue.CONCLUSION Acupoint application of BTD may promote the deformation and migration of myeloid dendritic cells in skin tissue,enhance antigen presentation,activate immune function of body,exhibit low toxic/side effects and play some regulatory role in an imbalance of Th1/Th2.
