天麻素胶囊与卡马西平联合治疗对PTN患者临床疗效、瞬目反射、疼痛程度、睡眠质量的影响

Effects of gastrodin capsule combined with carbamazepine on clinical efficacy,blinking reflex,pain degree and sleep quality of PTN patients

目的:探讨天麻素胶囊与卡马西平联合治疗对原发性三叉神经痛(Primary trigeminal neuralgia,PTN)患者临床疗效、瞬目反射、疼痛程度、睡眠质量的影响。方法:选取2021年6月至2023年6月虞城县人民医院收治的78例PTN患者作为研究对象,遵循随机对照原则,将所有患者分为中药联合组和西药组,各39例。西药组采用卡马西平治疗,中药联合组在西药组的基础上联合天麻素胶囊治疗。治疗1 m后,比较两组疗效、瞬目反射(R1、R2及R2′潜伏期)、血清指标神经生长因子(Nerve growth factor,NGF)、肿瘤坏死因子α(Tumor necrosis factorα,TNF-α)、疼痛程度、睡眠质量、不良反应发生情况。结果:治疗后,中药联合组总有效率明显高于西药组(P<0.05),中药联合组瞬目反射R1、R2及R2′潜伏期均明显短于西药组,中药联合组血清NGF水平、TNF-α水平、视觉模拟量表(Visual analog scale,VAS)评分、匹兹堡睡眠质量指数量表(Pittsburgh sleep quality index,PSQI)评分均低于西药组(P<0.05)。两组患者不良反应总发生率无明显差异(P>0.05)。结论:天麻素胶囊与卡马西平联合治疗PTN患者,可有效提高临床疗效,减轻瞬目反射和炎性反应程度,降低疼痛程度,提高睡眠质量,具有较高的用药安全性。

Objective:To investigate the effects of gastrodin capsule combined with carbamazepine on clinical efficacy,blinking reflex,pain degree and sleep quality in patients with primary trigeminal neuralgia(PTN).Methods:All 78 patients with PTN admitted to Yucheng County People's Hospital from June 2021 to June 2023 were collected as research objects.According to the principle of randomized control,all patients were divided into traditional Chinese medicine(TCM)combination group and western medicine group,39 cases in each group.The western medicine group was treated with carbamazepine,and the TCM combination group was treated with gastrodin capsule on the basis of the western medicine group.After 1 month of treatment,the curative effect,blink reflex(R1,R2,and R2’incubation period),serum indexes nerve growth factor(NGF),tumor necrosis factorα(TNF-α),pain level,sleep quality,occurrence of adverse reactions were compared between the two groups.Results:After treatment,the total effective rate of TCM combination group(94.87%)was significantly higher than that of western medicine group(79.49%)(P<0.05),blinking reflex R1,R2 and R2’latency in TCM combination group were shorter than those in western medicine group,and serum levels of NGF and TNF-α,visual analog scale(VAS)score and Pittsburgh sleep quality index(PSQI)score in TCM combination group were lower than those in western medicine group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Gastrodin capsule combined with carbamazepine in the treatment of PTN patients can effectively improve clinical efficacy,reduce blinking reflex and inflammatory response,reduce pain,improve sleep quality,and have high drug safety.
