经肛括约肌间切开术治疗肛瘘的临床研究进展

Clinical research progress of transanal opening of intersphincteric space in the treatment of anal fistula

肛瘘是一种常见的肛肠外科疾病,多由肛腺感染引起。外科手术是目前肛瘘主流的治疗方式,但同时兼顾保护肛门括约肌功能和提高治愈率是肛瘘手术发展的难题。经肛括约肌间切开术是治疗肛瘘的保留括约肌术式之一,全程保护肛门括约肌功能,并发症少,是当今肛瘘治疗的一种微创术式,受到广泛关注。本文通过整理和总结近年来经肛括约肌间切开术治疗肛瘘的相关文献,基于肛瘘发病机制及经肛括约肌间切开术的理论基础,综合阐述了近年来经肛括约肌间切开术治疗肛瘘的研究及临床进展,从而进一步推动经肛括约肌间切开术的优化与发展。

Anal fistula is a common anorectal surgical disease,mostly caused by anal gland infection.Surgical intervention is currently the mainstream treatment for anal fistula,but balancing the protection of anal sphincter function and improving the cure rate remains a challenge in the development of anal fistula surgery.Transanal opening of the intersphincteric space(TROPIS)is one of the sphincter-preserving surgical procedures for the treatment of anal fistula.It fully protects the function of the anal sphincter with few complications,making it a minimally invasive surgical approach for the treatment of anal fistula that has received widespread attention.This article summarizes the relevant literature on the treatment of anal fistula with TROPIS in recent years.Based on the pathogenesis of anal fistula and the theoretical foundation of TROPIS,it comprehensively elaborates the research and clinical progress of TROPIS in the treatment of anal fistula in recent years,thereby further promoting the optimization and development of TROPIS.
