潜阳封髓丹配合集束化管理对老年髋部骨折术后谵妄的影响

Effect of Qianyang Fengsui Dan combined with bundle management on postoperative delirium in elderly patients with hip fractures

目的探讨潜阳封髓丹配合集束化管理在老年髋部骨折术后谵妄并发症中的临床应用效果。方法选取2022年7月至2023年7月江西省新余市中医院行手术治疗的60例老年髋部骨折患者作为研究对象,按照随机数字表法分为观察组(30例)和对照组(30例)。两组均接受术后集束化管理,对照组进行常规术后干预,观察组术后接受潜阳封髓丹治疗。比较两组患者手术前后中医症状积分、疼痛程度、生活质量的变化情况以及术后并发症发生情况的差异。结果术后1周,观察组疼痛评分、肿胀评分、气滞血瘀评分、忧郁伤神评分均低于对照组,差异有统计学意义(P<0.05);术后5 d,观察组的视觉模拟评分法(VAS)评分低于对照组,差异有统计学意义(P<0.05);观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05);术后1个月,观察组36条简明健康状况调查表(SF-36)评分高于对照组,差异有统计学意义(P<0.05)。结论潜阳封髓丹配合集束化管理能够改善老年髋部骨折术后中医症状积分、疼痛程度和生活质量,降低谵妄发生率,值得临床推荐。

Objective To explore the clinical application effect of Qianyang Fengsui Dan combined with bundle manage-ment in the management of postoperative delirium complications in elderly patients with hip fractures.Methods A total of 60 elderly patients with hip fractures underwent surgical treatment at Xinyu Traditional Chinese Medicine Hospital in Jiangxi Provincefrom July 2022 to July 2023 were selected as the study objects and they were divived into observation group(30 cases)and control group(30 cases)according to random number table method.Both groups received postoperative bun-dle management,the control group received routine postoperative intervention,and the observation group received Qianyang Fengsui Dan treatment.The changes in traditional Chinese medicine symptom scores,pain levels,quality of life,and post-operative complications between two groups of patients before and after surgery were compared.Results The pain score,swelling score,qi stagnation and blood stasis score,and depression and injury score of the observation group were lower than those of the control group one week after surgery,and the differences were statistically significant(P<0.05).The visual ana-logue scale(VAS)score of the observation group was lower than that of the control group at 5 days after surgery,and the dif-ference was statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The score of the 36 short form health sta-tus survey(SF-36)of the observation group was higher than that of the control group one month after surgery,and the differ-ence was statistically significant(P<0.05).Conclusion The combination of Qianyang Fengsui Dan and bundle management can improve the postoperative TCM symptom score,pain level,and quality of life in elderly patients with hip fractures,re-duce the incidence of delirium,and is worthy of clinical recommendation.
