呼吸肌训练干预对脑损伤长期卧床患者肺康复效果及生存质量的影响

Effects of respiratory muscle training intervention on pulmonary rehabilitation and quality of life of patients with long-term bedridden patients with brain injury

目的探讨呼吸肌训练干预在脑损伤长期卧床患者中的应用效果。方法选择2020年1月至2021年1月就诊于本院的96例病情稳定的脑损伤长期卧床患者作为研究对象,采用随机数字表法将其分为A组、B组,每组48例。A组行常规康复训练,B组在A组基础上开展呼吸肌训练干预。比较两组的干预效果。结果干预后,B组的最大吸气压(MIP)及吸气峰流速(PIF)高于A组(P<0.05)。干预后,B组的用力肺活量(FVC)、每分钟最大通气量(MVV)及第1秒用力呼气量(FEV_(1))高于A组(P<0.05);B组康复期间的肺部感染发生率低于A组(P<0.05)。干预后,B组的生理、心理及社会关系评分高于A组(P<0.05)。结论呼吸肌训练干预能够促进脑损伤长期卧床患者呼吸肌功能及肺功能有效改善,减少肺部感染,并可提高患者的生存质量。

Objective To explore the application effect of respiratory muscle training intervention in long-term bedridden patients with brain injury.Methods A total of 96 long-term bedridden patients with stable brain injury who were treated in our hospital from January 2020 to January 2021 were selected as the research objects.The patients were divided into group A and group B by random number table method,with 48 cases in each group.The group A received routine rehabilitation training,and the group B received respiratory muscle training intervention on the basis of the group A.The intervention effects of the two groups were compared.Results After intervention,the maximum inspiratory pressure(MIP)and peak inspiratory flow(PIF)in the group B were higher than those in the group A(P<0.05).After intervention,the forced vital capacity(FVC),maximum voluntary ventilation(MVV)and force expiratory volume in one second(FEV_(1))in the group B were higher than those in the group A(P<0.05);the incidence of pulmonary infection during rehabilitation in the group B was lower than that in the group A(P<0.05).After intervention,the physiological,psychological and social relationship scores of the group B were higher than those of the group A(P<0.05).Conclusion Respiratory muscle training intervention can promote the effective improvement of respiratory muscle function and lung function in long-term bedridden patients with brain injury,reduce pulmonary infection,and improve the quality of life of patients.
