身痛逐瘀汤加减对胸腰椎骨质疏松性压缩性骨折患者术后骨代谢指标及腰椎功能的影响

The effect of modified Shentong Zhuyu Tang on postoperative bone metabolism indicators and lumbar spine function in patients with thoracolumbar osteoporotic compression fractures

目的探讨身痛逐瘀汤加减对胸腰椎骨质疏松性压缩性骨折(OVCF)患者术后骨代谢指标及腰椎功能的影响。方法择取2021年10月-2023年10月江西省泰和县中医院接诊的80例OVCF患者,随机分成实验组、常规组,每组40例。常规组术后予骨化三醇胶丸、碳酸钙D3片治疗,实验组在其基础上予身痛逐瘀汤治疗,对比两组疼痛程度、骨代谢指标、腰椎功能与血液流变学指标。结果治疗后,两组的视觉模拟评分法(VAS)评分均降低,且实验组VAS评分更低,有统计学差异(P<0.05);治疗后,两组骨钙素(BGP)均增加,I型胶原氨基端延长肽(PINP)与Ⅰ型胶原交联C-末端肽(β-CTX)均降低,且实验组BGP、PINP与β-CTX改善更优,有统计学差异(P<0.05);治疗后,两组Oswestry功能障碍指数(ODI)评分均降低,且实验组ODI评分更低,有统计学差异(P<0.05);治疗后,两组红细胞压积、全血比黏度与血浆比黏度均降低,且实验组红细胞压积、全血比黏度与血浆比黏度更低,有统计学差异(P<0.05)。结论身痛逐瘀汤加减用于OVCF患者术后可改善骨代谢与血液流变学,减轻疼痛,改善腰椎功能。

Objective To explore the effect of modified Shentong Zhuyu Tang on postoperative bone metabolism indicators and lumbar spine function in patients with thoracolumbar osteoporotic compression fractures(OVCF).Method 80 OVCF patients admitted to the hospital from October 2021 to October 2023 were randomly divided into an experimental group and a control group,with 40 patients in each group.The control group was treated with calcitriol capsules and calcium carbonate D3 tablets after surgery,while the experimental group was treated with Shentong Zhuyu Tang on the basis of it.The pain level,bone metabolism indicators,lumbar spine function,and hemorheological indicators were compared between the two groups.Results After treatment,the visual analogue scale(VAS)scores of both groups decreased,and the VAS score of the experimental group was lower,with a statistical difference(P<0.05);after treatment,both groups of osteocalcin(BGP)increased,and type I collagen amino terminal elongation peptide(PINP)and type I collagen crosslinked C-terminal peptide(β-CTX)levels were all reduced,and BGP,PINP,andβ-CTX showed better improvement with statistical differences(P<0.05);after treatment,both Oswestry Dysfunction Index(ODI)scores decreased in both groups,and the experimental group had a lower ODI score,with a statistically significant difference(P<0.05);after treatment,the hematocrit,whole blood viscosity,and plasma viscosity of both groups decreased,and the experimental group had lower hematocrit,whole blood viscosity,and plasma viscosity,with statistical differences(P<0.05).Conclusion Modified Shentong Zhuyu Tang can improve bone metabolism and hemorheology,alleviate pain,and improve lumbar spine function in OVCF patients after surgery.
