摘要 尊敬的尚翔、果伟老师:感谢你们对拙作《度洛西汀致神经阻滞剂恶性综合征》一文的关注,关于对本文所讨论内容,回复如下:(1)根据美国精神病协会《精神疾病诊断与统计手册》[1]、Hunter标准[2]中关于神经阻滞剂恶性综合征的诊断标准,首要条件是患者服用了多巴胺受体拮抗剂或停用多巴胺受体激动剂,这是与5‑羟色胺综合征进行鉴别诊断的基础。该例患者应用了度洛西汀抗抑郁药(非抗精神病药),抗抑郁药物引起神经阻滞剂恶性综合征较少见,但结合既往相关案例报道[3‑5]与患者的症状表现、实验室检查结果,考虑患者因应用度洛西汀引起神经阻滞剂恶性综合征。5-羟色胺综合征患者在眼科检查可发现瞳孔散大和眼震,应加以鉴别,文中对肢体抖动、肌力和肌张力及眼部症状的描述需细化。
