摘要
目的 探讨超声可视化教学模式在麻醉科住院医师规范化培训(以下简称为住培)教学中的应用效果。方法 选取2021-08/2022-08在作者医院麻醉科住培数学基地38名培训学员为研究对象。采用随机数字表法分为观察组(n=19)和对照组(n=19)。对照组采用传统的教学方法,观察组在传统教学基础上采用超声可视化教学方法。对入科学员进行中心静脉、外周动脉以及椎管内穿刺的理论知识授课和技能操作培训。比较两组学员理论考核成绩,首次技能操作成功率、首次穿刺成功所需病例数、首次穿刺成功用时,前10次穿刺失败病例数、前10次穿刺平均用时以及前10次穿刺发生并发症的病例数。结果 两组学员的理论考核成绩比较差异无统计学意义(P>0.05)。观察组学员首次椎管内穿刺成功率较对照组高,中心静脉、椎管内穿刺首次穿刺成功用时较对照组明显降低,椎管内穿刺首次穿刺成功所需病例数、前10次中心静脉穿刺和椎管内穿刺失败病例数较对照组少,前10次中心静脉和椎管内穿刺平均用时较对照组明显缩短(P均<0.05)。结论 超声可视化模式下的教学应用在麻醉科住培学员中,可以提高中心静脉、椎管内穿刺等有创操作的穿刺成功率,缩短穿刺所需时间,可提高佳培学员的培训质量和岗位胜任力,值得大力推广。
Objective To investigate the effects of ultrasound visualization teaching mode on the standardized train-ing of residents in anesthesiology department.Methods A total of 38 trainees who were trained in the resident companion teaching base of the department of anesthesiology in author's hospital from August,2021 to August,2022 were selected as study subjects.The trainees were divided into observation group(n=19)and control group(n=19)according to the random number table method.The control group adopted the traditional teaching method,and the observation group adopted the ultrasound visualization teaching method on the base of traditional teaching method.The theoretical knowl-edge and skills training of central vein puncture,peripheral artery puncture and intraspinal puncture were given to the re-cruited trainees.The theoretical knowledge assessment performance of the trainees,the success rate of the first opera-tion,the cases and the time required for the first successful operation,also the failure cases,the average time and the ca-ses of complications of the first 10 punctures were compared between the two groups.Results There was no significant difference in theoretical performance of trainees between the two groups(P>0.05).The success rate of the first intraspi-nal puncture in the observation group was higher than that in the control group,the time of the first successful operation in the central vein puncture and intraspinal puncture was significantly lower than that in the control group,the number of successful cases of the first puncture in the intraspinal puncture and the number of failed cases of central vein puncture and intraspinal puncture in the first 10 puncture was less than those in the control group,the average time of first 10 times central vein puncture and intraspinal puncture was significantly shorter than that in the control group(all P<0.05).Conclusion The application of ultrasound visualization teaching mode on the standardized training of residents in anesthe-siology department can improve the success rate of puncture in central vein puncture and intraspinal puncture,shorten the time for operation,and improve the training quality and post competence of residents in standardized training,which is worth promoting vigorously.
