丙泊酚联合依托咪酯对高龄患者无痛胃镜检查呼吸、循环系统及认知功能的影响

Impact of Propofol Combined with Etomidate on Respiratory,Circulatory System,and Cognitive Function in Elderly Patients Undergoing Painless Gastroscopy

目的:探析丙泊酚联合依托咪酯对高龄患者无痛胃镜检查呼吸、循环系统及认知功能的影响。方法:将在本院接受治疗的100例高龄无痛胃镜检查患者按抽签法分为研究组和对照组,各50例;对照组采用依托咪酯进行麻醉,研究组采用依托咪酯联合丙泊酚进行麻醉;对两组患者的麻醉效果、呼吸循环系统指标、认知功能和不良反应发生率进行比较。结果:研究组麻药诱导时间、术后苏醒时间、定向力恢复时间以及麻醉阻滞效果评分均低于对照组(P<0.05);检查时两组收缩压(SBP)、舒张压(DBP)、血氧饱和度(SpO_(2))、心率(HR)、呼吸频率(RR)较检查前均有所下降,且研究组低于对照组(P<0.05);研究组认知功能较对照组更高,不良反应发生率较对照组更低(P<0.05)。结论:针对临床行无痛胃镜检查的高龄患者,检查中采用丙泊酚联合依托咪酯麻醉具有较好的应用效果,能有效提升麻醉效果,改善循环呼吸系统指标,促进患者认知功能的恢复,还能减少不良反应的发生,值得推广。

Objective:To investigate the effects of propofol combined with etomidate on respiratory and circulatory systems,as well as cognitive function in elderly patients undergoing painless gastroscopy.Methods:A total of 100 elderly patients scheduled for painless gastroscopy were randomly divided by lottery into a study group and a control group,with 50 patients in each group.The control group received anesthesia with etomidate alone,while the study group was anesthetized with a combination of etomidate and propofol.Anesthesia effectiveness,respiratory and circulatory system parameters,cognitive function,and the incidence of adverse reactions were compared between the two groups.Results:Induction time,recovery time after surgery,time to orientation recovery,and anesthesia blocking effect scores were all lower in the study group compared to the control group(P<0.05).During the examination,systolic blood pressure(SBP),diastolic blood pressure(DBP),oxygen saturation(SpO_(2)),heart rate(HR),and respiratory rate(RR)decreased in both groups compared to pre-examination levels,with the study group exhibiting lower values than the control group(P<0.05).The study group displayed better cognitive function and a lower incidence of adverse reactions than the control group(P<0.05).Conclusion:For elderly patients undergoing painless gastroscopy,the use of propofol combined with etomidate for anesthesia shows favorable clinical application results.This combination effectively enhances anesthesia efficiency,improves circulatory and respiratory parameters,promotes cognitive function recovery,and reduces the occurrence of adverse reactions,thus warranting wider application.
