三种干预方案治疗老年缺血性脑卒中患者便秘的效果比较

Comparison of the Efficacy of Three Intervention Regimens in Treating Constipation in Elderly Patients with Ischemic Stroke

目的:分析三种干预方案治疗老年缺血性脑卒中(IS)患者便秘的实际效果。方法:按照简单随机法将92例患者分为A、B、C三组;A组(30例)患者进行膳食纤维联合双歧杆菌干预,B组(31例)患者进行球囊扩张联合盆底肌锻炼干预,C组(31例)患者进行低频电穴位刺激干预;比较三组患者的临床疗效、便秘症状评分及心理状态。结果:C组总有效率(96.77%)高于B组(67.74%)与A组(66.67%)(P<0.05),A组与B组之间差异无统计学意义(P>0.05);干预后C组患者便秘症状各维度(排便困难、粪便形状、排便时间、下坠不尽感、排便频率、腹胀)评分均较A组和B组低(P<0.05),A组与B组之间差异无统计学意义(P>0.05);干预后C组患者的SAS、SDS评分较A组和B组低(P<0.05),A组与B组之间差异无统计学意义(P>0.05)。结论:与其他两种方式相比,低频电穴位刺激应用于老年IS便秘患者中能有效提高治疗效果,改善便秘症状及患者的心理状态。

Objective:To analyze the practical effects of three intervention approaches on treating constipation in elderly patients with ischemic stroke(IS).Methods:Ninety-two patients were randomly allocated into three groups(Group A,B,and C)using simple randomization;Group A(30 patients)received dietary fiber combined with bifidobacterium intervention,Group B(31 patients)underwent balloon dilation combined with pelvic floor muscle exercises,and Group C(31 patients)received low-frequency electroacupuncture stimulation.Clinical efficacy,constipation symptom scores,and psychological status were compared among the three groups.Results:Group C demonstrated a significantly higher overall response rate(96.77%)compared to Group B(67.74%)and Group A(66.67%)(P<0.05),with no significant difference between Groups A and B(P>0.05).Following intervention,Group C showed lower scores across all dimensions of constipation symptoms(difficulty in defecation,stool form,defecation time,feeling of incomplete evacuation,defecation frequency,and abdominal distension)compared to Groups A and B(P<0.05),with no significant difference between Groups A and B(P>0.05).Furthermore,post-intervention,Group C had lower SAS and SDS scores than Groups A and B(P<0.05),with no significant difference observed between Groups A and B(P>0.05).Conclusion:Compared to the other two methods,low-frequency electroacupuncture stimulation proved more effective in improving therapeutic outcomes,relieving constipation symptoms,and enhancing the psychological state of elderly IS patients with constipation.
