神经内镜血肿清除术治疗重型脑室出血并铸型的作用分析

Analysis of the Role of Neuroendoscopic Hematoma Evacuation in the Treatment of Severe Intraventricular Hemorrhage with Intraventricular Clot Formation

摘要 目的:明确神经内镜血肿清除术治疗重型脑室出血并铸型的临床价值。方法:将本院96例患者采用随机数字表法分为对照组和观察组各48例;对照组采用侧脑室外引流术式,观察组采用神经内镜血肿清除术;比较两组患者术前术后mGS评分,术后尿激酶用量、引流管留置时间、并发症和治疗效果。结果:术前两组患者mGS评分无统计学意义,术后差异显著,观察组低于对照组(P<0.05);术后观察组尿激酶用量、引流管留置时间和并发症发生率均低于对照组(P<0.05),治疗总有效率高于对照组(P<0.05)。结论:对重型脑室出血铸型患者采用神经内镜血肿清除术,有利于改善患者临床症状,降低并发症发生率。 Objective:To clarify the clinical value of neuroendoscopic hematoma evacuation in treating severe intraventricular hemorrhage with intraventricular clot formation.Methods:Ninety-six patients from our hospital were randomly divided into a control group and an observation group(n=48 each)using a random number table.The control group underwent lateral ventricular drainage,while the observation group received neuroendoscopic hematoma evacuation.Preoperative and postoperative mGS scores,postoperative urokinase dosage,catheter retention time,complications,and treatment efficacy were compared between the two groups.Results:There was no significant difference in preoperative mGS scores between the two groups,but postoperative scores were significantly lower in the observation group compared to the control group(P<0.05).Postoperatively,the observation group used less urokinase,had shorter catheter retention times,and experienced a lower rate of complications compared to the control group(all P<0.05).Moreover,the total effective rate of treatment was higher in the observation group than in the control group(P<0.05).Conclusion:Neuroendoscopic hematoma evacuation for patients with severe intraventricular hemorrhage and clot formation is beneficial for improving clinical symptoms and reducing the incidence of complications.
