手功能训练器应用于颈椎病术后患者对其上肢握力、运动功能的影响

Effects of Hand Function Training Device on Upper Limb Grip Strength and Motor Function in Patients with Cervical Spondylosis after Surgery

摘要 目的:探讨手功能训练器应用于颈椎病术后患者对其上肢握力、运动功能的影响.方法:选取2018年1月-2020年12月我院收治的90例颈椎病患者为研究对象,均采用手术治疗,随机分为对照组、研究组,每组45例.对照组采用常规康复训练,研究组采用手功能训练器康复训练,总疗程12周.比较2组不同训练时间点上肢握力、肌力水平,上肢麻木、运动功能及表面肌电特征.结果:训练4、8、12周后,2组上肢握力均大于训练前,且研究组大于对照组,差异有统计学意义(P<0.05).训练4、8、12周后,2组上肢腕部、肘部、肩部肌力均高于训练前,且研究组高于对照组,差异有统计学意义(P<0.05).训练12周后,2组上肢麻木程度均较训练前减轻,且研究组轻于对照组,差异有统计学意义(P<0.05).训练4、8、12周后,2组上肢运动功能评分均高于训练前,且研究组高于对照组,差异有统计学意义(P<0.05).训练4、8、12周后,2组肱二头肌与肱三头肌RMS值及协同收缩比均较训练前降低,且研究组低于对照组,差异有统计学意义(P<0.05).结论:对于颈椎病术后患者在常规康复训练基础上进行手功能训练器训练,可有效改善其上肢握力、肌力及麻木水平,从而改善上肢运动功能. Objective:To explore the clinical effect of hand function training device on the grip strength and motor function of upper limbs in patients with cervical spondylosis after operation.Methods:90 patients with cervical spondylosis admitted to our hospital from January 2018 to December 2020 were selected as the research objects.All patients were treated with surgery and were randomly divided into a control group and an study group,with 45 cases in each group.The control group was treated with routine rehabilitation training,and the study group was treated with hand function training device rehabilitation training.The total course of treatment was 12 weeks.The upper limb grip strength,muscle strength,upper limb numbness,motor function and surface electromyography characteristics were compared between the two groups at different training time points.Results:After 4,8 and 12 weeks of training,the upper limb grip strength of the two groups was higher than that before training,and the study group was higher than the control group,the differences were statistically significant(P<0.05).After 4,8 and 12 weeks of training,the muscle strength of the wrist,elbow and shoulder of the two groups were higher than those before training,and the muscle strength of the study group was higher than that of the control group,the differences were statistically significant(P<0.05).After 12 weeks of training,the degree of upper limb numbness in both groups was reduced compared with that before training,and the degree of upper limb numbness in the study group was lighter than that in the control group,and the difference was statistically significant(P<0.05).After 4,8 and 12 weeks of training,the upper limb motor function scores of the two groups were higher than those before training,and the scores of the study group were higher than those of the control group,the differences were statistically significant(P<0.05).After 4,8 and 12 weeks of training,the RMS value and cocontraction ratio of biceps and triceps brachii in the two groups were lower than those before training,and the study group was lower than the control group,the differences were statistically significant(P<0.05).Conclusion:The hand function training on the basis of routine rehabilitation training can effectively improve the grip strength,muscle strength and numbness level of upper limbs,so as to improve the motor function of upper limbs.
