宣肺清解汤治疗肺结节的临床疗效

Clinical Efficacy of Xuanfei Qingjie Decoction in Treatment of Pulmonary Nodules

目的评价宣肺清解汤治疗肺结节的临床疗效。方法选择2021年9月至2022年10月北京中医药大学附属护国寺中医医院门诊招募的66例肺结节患者为研究对象,依据随机数字表法分为治疗组和对照组,每组33例。对照组仅进行健康宣教,在观察周期结束后进行CT随访复查;治疗组在对照组的基础上应用宣肺清解汤治疗,每次口服200 ml,每日2次,疗程为3个月。比较两组患者治疗前后结节最大直径、结节危险程度分级、中医体质症状积分,以及治疗后的临床疗效。结果与治疗前比较,治疗后对照组结节最大直径无明显变化(P>0.05),治疗组结节最大直径减小(P<0.05);治疗后,两组患者结节最大直径比较差异无统计学意义(P>0.05)。治疗后,治疗组低危患者比例增加,两组结节危险程度分级比较差异有统计学意义(P<0.01)。治疗后,两组患者中医体质症状积分均低于治疗前(P<0.01),且治疗组低于对照组[(3.6±1.4)分比(4.4±1.5)分](P<0.05)。治疗后,治疗组结节缩小总有效率、中医证候总有效率高于对照组[20.00%(6/30)比3.20%(1/31)、66.67%(20/30)比35.48%(11/31)](χ^(2)=6.667,P=0.010;χ^(2)=5.931,P=0.015)。结论宣肺清解汤能够改善肺结节患者的体质症状,缩小部分患者肺结节直径,且临床疗效显著。

Objective To evaluate the clinical efficacy of Xuanfei Qingjie decoction in the treatment of pulmonary nodules.Methods A total of 66 patients with pulmonary nodules recruited at the outpatient department of Huguosi Hospital of Traditional Chinese Medicine(TCM)Affiliated to Beijing University of Chinese Medicine from Sep.2021 to Oct.2022 were divided into a treatment group(33 cases)and a control group(33 cases)according to the random number table method.The control group only received health education and CT follow-up review after the end of the observation period;the treatment group was given Xuanfei Qingjie decoction with 200 ml orally twice a day for a course of 3 months on the basis of the control group′s regimen.The maximum diameter of nodules,the risk degree of nodules,the score of TCM constitution symptoms before and after treatment,and the clinical efficacy after treatment of the two groups were compared.Results Compared with before treatment,the maximum nodule diameter in the control group had no significant change after treatment(P>0.05),and the maximum nodule diameter in the treatment group decreased(P<0.05).After treatment,there was no significant difference in the maximum diameter of nodules between the two groups(P>0.05).After treatment,the proportion of low-risk patients in the treatment group increased,and there was a statistically significant difference in the classification of nodule risk between the two groups(P<0.01).After treatment,the scores of TCM constitution symptoms in both groups were lower than before treatment(P<0.01),and the scores in the treatment group were lower than those in the control group(3.6±1.4 vs 4.4±1.5)(P<0.05).After treatment,the total effective rated of nodule reduction and TCM syndromes in the treatment group were higher than those in the control group[20.00%(6/30)vs 3.20%(1/31),66.67%(20/30)vs 35.48%(11/31)](χ^(2)=6.667,P=0.010;χ^(2)=5.931,P=0.015).Conclusion Xuanfei Qingjie decoction can improve the physical symptoms of the patients,reduce the diameter of some patients′pulmonary nodules,and has significant clinical efficacy.
