调脊通督针法联合腕踝针治疗腰椎间盘突出症的临床研究

Clinical Study of Tiaoji Tongdu Acupuncture Combined with Wrist-Ankle Acupuncture in Treatment of LDH

目的:探究调脊通督针法联合腕踝针治疗腰椎间盘突出症(LDH)的临床疗效。方法:将本院2021年1月—2023年6月收治的94例寒湿痹阻型LDH患者,随机分为对照组47例观察组,对照组给予腕踝针治疗,观察组给予调脊通督针法联合腕踝针治疗。记录治疗4周后两组患者的临床疗效;记录治疗前、治疗2周、4周后的腰部疼痛情况[视觉模拟评分(VAS)];记录治疗前、治疗4周后的中医症候积分、腰椎功能[日本骨科学会评分法(JOA)、Oswestry功能障碍指数(ODI)]和腰椎活动度。结果:治疗后,观察组临床疗效显著高于对照组,差异有统计学意义(P<0.05);两组患者的VAS评分均显著降低,且观察组低于同期对照组,差异有统计学意义(P<0.05);治疗4周后,两组患者中医症候评分、ODI评分显著降低,且观察组低于对照组,差异有统计学意义(P<0.05),两组患者JOA评分、腰椎活动度显著升高,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:调脊通督针法联合腕踝针治疗LDH患者疗效显著,可缓解患者腰部疼痛,改善腰椎功能和活动情况。

Objective:To explore the efficacy of Tiaoji Tongdu acupuncture combined with wrist-ankle acupuncture in treatment of lumbar disc herniation(LDH).Methods:94 LDH cases of cold-dampness obstruction pattern were randomly divided into the control group and the observation group,with 47 cases in each group.The control group was treated with wrist-ankle acupuncture;on which basis,the observation group was treated with Tiaoji Tongdu acupuncture.Clinical efficacy was recorded after 4 weeks of the treatment.Lumbar pain status[Visual Analogue Scale(VAS)]was observed before the treatment,after 2 w and 4 w of the treatment;TCM symptoms scores,lumbar function[Japanese Orthopedic Association score(JOA),Oswestry Disability Index(ODI)],and lumbar mobility were observed before the treatment and after 4 w of the treatment in the two groups.Results:After 4 weeks of the treatment,the total effective rate in the observation group was significantly higher than that in the control group(P<0.05).The VAS score was significantly decreased in the two groups,and the VAS score in the observation group was lower than that in the control group at the same time point(P<0.05).The scores of TCM symptorns and ODI were significantly decreased in the two groups,and the scores were lower in the observation group than those in the control group(P<0.05).The JOA score and lumbar mobility were increased significantly in both groups,and the indicators were higher in the observation group than those in the control group(P<0.05).Conclusion:Tiaoji Tongdu acupuncture combined with wrist-ankle acupuncture has a significant effect in treatment of LDH,and it can relieve lumbar pain,and improve lumbar function and activity.
