基于CICARE沟通模式的健康教育对腰椎间盘突出症术后患者的影响

摘要 目的:探讨基于CICARE沟通模式的健康教育对腰椎间盘突出症(LDH)术后患者心理状态、自我管理能力及生活质量的影响。方法:选择2023年2—8月在山东大学齐鲁医院住院治疗的198例LDH患者作为研究对象,剔除41例药物保守治疗和1例神经阻滞治疗,共纳入156例手术治疗患者,根据随机数表法将其分为常规组和观察组,各78例。常规组实施院内常规健康教育,观察组采用基于CICARE沟通模式的健康教育进行干预,随访3个月。比较干预前后两组心理状态、自我管理能力及生活质量情况。结果:干预前,两组抑郁自评量表(SDS)评分、焦虑自评量表(SAS)评分、自我管理能力评分、生活质量评分比较,差异无统计学意义(P>0.05)。干预后,两组SDS评分、SAS评分均低于干预前,认知症状管理、与医生沟通、运动锻炼评分及生活质量各指标评分均高于干预前,且观察组SDS评分、SAS评分均低于常规组,认知症状管理、与医生沟通、运动锻炼评分及生活质量各指标评分均高于常规组,差异有统计学意义(P<0.05)。结论:基于CICARE沟通模式的健康教育,能够有效改善LDH患者术后焦虑及抑郁等心理状态,提高其自我管理能力及生活质量水平,对LDH患者预后康复具有一定的积极作用。 Objective:To explore the effects of health education based on CICARE communication model on postoperative mental state,self-management ability and quality of life of patients with lumbar disc herniation(LDH).Method:A total of 198 patients with LDH hospitalized in Qilu Hospital of Shandong University from February to August 2023 were included in this study,and a total of 156 patients receiving surgical treatment were included after excluding 41 cases of conservative drug treatment and 1 case of nerve block treatment.They were divided into conventional group and observation group according to random number table method,with 78 cases in each group.The conventional group was given hospital routine health education,and the observation group was followed up for 3 months by health education based on CICARE communication model.The psychological state,self-management ability and quality of life of the two groups were compared before and after the intervention.Result:Before intervention,there were no significant differences in selfrating depression scale(SDS)score,self-rating anxiety scale(SAS)score,self-management ability score and life quality score between the two groups(P>0.05).After intervention,SDS scores and SAS scores of both groups were lower than those before intervention,scores of cognitive symptom management,communication with doctors,exercise scores and quality of life indexes were higher than those before intervention,and SDS score and SAS score of the observation group were lower than those of the conventional group,the scores of cognitive symptom management,communication with doctors,exercise and quality of life were all higher than those of conventional group,the differences were statistically significant(P<0.05).Conclusion:Health education based on CICARE communication model can effectively improve postoperative anxiety and depression and other psychological states of LDH patients,improve their self-management ability and life quality,and have certain positive effects on the prognosis and rehabilitation of LDH patients.
